Introdução: Com o aumento da prevalência da doença renal crônica (DRC) associada à maior expectativa de vida e maior número de comorbidades, faz-se necessário promover melhor qualidade de vida ao paciente idoso com DRC avançada, o que pode ser alcançado com o tratamento conservador, definido como tratamento da doença sem dialise, visando retardar a progressão da perda da função renal residual, manejo de sintomas, minimizar intercorrências e oferecer suporte psicossocial, por meio de uma abordagem multiprofissional. O ambulatório multiprofissional de cuidados paliativos para portadores de DRC estágio V foi iniciado em 2018 em hospital do interior de SP. Objetivo: Avaliar o perfil epidemiológico dos pacientes em seguimento ambulatorial e opção por tratamento conservador da DRC avançada. Métodos: Coorte retrospectiva com pacientes adultos com DRC avançada e opção por tratamento conservador de jan/2018 a jun/2024. Resultados: Foram incluídos 30 pacientes, com idade 83,8 ± 10 anos, predominância do sexo feminino (56,7%) e com taxa de filtração glomerular de 15,1 ± 5,3. A doença de base foi nefroesclerose hipertensiva (30%), seguido de doença renal do diabetes (23,3%), nefropatia obstrutiva (20%) e sequela de injúria renal aguda (10%). As comorbidades mais prevalentes foram hipertensão arterial sistêmica (93,3%), insuficiência cardíaca (50%), diabetes melito (43,3%) e depressão (33,3%). A opção pelo tratamento conservador foi do paciente/familiares (53,3%). O escore PPS (Palliative Performance Scale) foi 60 (40-100), sendo que 10% dos pacientes pioraram PPS durante seguimento. O escore ESAS (Edmonton Symptom Assessment System) foi mantido (53,3%) ou melhorou (30%). Houve taxa de internação hospitalar de 46,6%, com 57,1% de reinternações. Apenas 10% dos pacientes mudaram de opinião devido a piora sintomática e iniciaram método dialítico (diálise peritoneal). As principais causas de dropout foram perda de seguimento durante a pandemia da Covid-19 (40%) e óbito (30%). Conclusão: O manejo conservador do paciente com DRC avançada, quando realizado por equipe multiprofissional e envolvendo planejamento compartilhado de ações, pode melhorar a qualidade de vida dos pacientes.
