Introdução: O fenótipo de fragilidade é caracterizado pela diminuição da capacidade do organismo em responder eficazmente a situações de estresse. Diversas escalas de fragilidade avaliam diferentes aspectos desse fenômeno. O presente estudo tem como objetivo analisar a relação entre a percepção da equipe médica e duas escalas de fragilidade amplamente utilizadas na área da geriatria: a Escala de Fragilidade do Cardiovascular Health Study (EFCHS) e a Escala de Edmonton. Métodos: Este estudo envolveu receptores de transplante renal do Hospital Israelita Albert Einstein, programa PROADI-SUS. Em nosso serviço, cada paciente é preferencialmente atendido pelo mesmo médico. A fragilidade foi avaliada em 276 indivíduos utilizando duas escalas diferentes: a Escala de Fragilidade do Cardiovascular Health Study (EFCHS, também conhecida como Escala de Fried) e a Escala de Edmonton (EFS). Após essa avaliação, cada médico da equipe foi questionado sobre a presença ou ausência de fragilidade ou pré-fragilidade nos pacientes que acompanha. Oito pacientes foram excluídos da análise devido à impossibilidade da equipe médica em classificá-los. A análise estatística foi realizada usando o software Jamovi. Resultados: Identificamos 175 pacientes frágeis ou pré-frágeis pela escala EFCHS e 166 pacientes frágeis ou pré-frágeis pela escala e Edmonton. A identificação do fenótipo de fragilidade pela equipe médica foi baixa: 33% (n=58) dos pacientes frágeis/pré-frágeis identificados pela escala de Fried foram classificados corretamente, enquanto que 35% (n=59) dos pacientes pacientes frágeis/pré-frágeis identificados pelo EFS foram classificados corretamente. A identificação dos robustos pela equipe médica foi alta: a equipe médica classificou corretamente 81% dos pacientes identificados como robustos pela escala de Fried e 82% dos pacientes identificados como robustos pelo EFS. Conclusão: A baixa sensibilidade da percepção clínica da síndrome de fragilidade pela equipe médica demonstra a complexidade desse fenótipo e reforça a necessidade da adoção de escalas padronizadas e validades para a correta identificação dessa síndrome.
