FENÓTIPO DE FRAGILIDADE EM RECEPTORES DE TRANSPLANTE RENAL: PREVALÊNCIA, CONDIÇÕES ASSOCIADAS E PRINCIPAIS DESFECHOS

Abstract
Introdução: O fenótipo de fragilidade (FF) está associado a piores desfechos após o transplante renal (TxR). Dados sobre essa síndrome na população brasileira são escassos. Este estudo teve como objetivo avaliar a prevalência e fatores associados ao fenótipo de fragilidade, bem como os desfechos em receptores de TxR. Métodos: Estudo prospectivo, incluindo adultos receptores de TxR entre 2019 e 2021. Pacientes foram avaliados < 24h antes do TxR. O FF foi avaliado pela escala de Fried. Atividades básicas(AVD) e instrumentais(AIVD) de vida diária foram avaliadas usando o score de Katz e a Escala de Lawton, respectivamente. A avaliação cognitiva utilizou o Montreal Cognitive Assessment (MoCA). Biomarcadores rotineiramente coletados no pré e pós-TxR foram analisados. Os pacientes foram divididos em dois grupos: frágeis ou pré-frágeis (Grupo F) e não frágeis (Grupo NF). Os desfechos de 1 ano foram avaliados. Resultados: Um total de 82 pacientes foram incluídos, predominantemente homens (80%), não-caucasianos (87%), 49±15 anos e IMC 24,9±4,3 Kg/m2. Trinta (36,6%) foram classificados como não frágeis, 28 (34,1%) pré-frágeis e 24 (29,3%) frágeis. Na escala de Fried, o Grupo F apresentou mais fraqueza (47% vs. 0%, p< 0,001), exaustão (38% vs. 3,3%, p< 0,001) e lentidão (37% vs. 0%, p< 0,001). Não houve diferença quanto à de perda de peso e gasto calórico. Em comparação com o Grupo NF, o Grupo F apresentou mais indivíduos hipertensos (90% vs. 70%, p=0,018), com doenças reumáticas (15% vs. 0%, p=0,024), com maior dependência para AVD (32% vs. 0%, p< 0,001) e AIVD (82% vs. 57%, p=0,014), e maior comprometimento cognitivo (65% vs. 37%, p=0,037). Não houve diferença entre os grupos quanto à incidência de função tardia do enxerto (57%) e rejeição aguda comprovada por biópsia (3,8%), sobrevida do paciente (98%) e do enxerto (98%). Pré-transplante, o grupo F tinha menor creatinina (7 vs. 9 mg/dL, p=0,044) e menor hemoglobina (11,9±2,7 vs. 13,4±1,8 g/dL, p=0,005). Após 1 ano, não houve diferença entre os grupos aos exames laboratoriais. Conclusão: A prevalência de fragilidade ou pré-fragilidade foi alta nesta amostra. O fenótipo de fragilidade foi associado à hipertensão, doenças reumáticas, menores níveis de creatinina e hemoglobina, bem como maior dependência em atividades básicas e instrumentais da vida diária, além de comprometimento cognitivo. Não foi observada associação entre fragilidade pré-TxR e os desfechos após o TxR.
