A RELAÇÃO ENTRE O NÍVEL DE CONHECIMENTO SOBRE O HPV E AS TAXAS DE VACINAÇÃO EM PAIS E ADOLESCENTES: UMA REVISÃO SISTEMÁTICA E META-ANÁLISE DE ESTUDOS BRASILEIROS

Abstract
Objetivo: Avaliar a associação entre o nível de conhecimento sobre o Papilomavírus Humano (HPV) e a adesão à vacinação contra o HPV entre pais e adolescentes no Brasil. Métodos: Foi conduzida uma revisão sistemática e meta-análise seguindo as recomendações PRISMA. As buscas foram realizadas nas bases de dados PubMed/MEDLINE, Embase, Scopus, Web of Science e Cochrane Library e complementadas pela análise de arquivos pré-selecionados. Foram incluídos estudos observacionais (transversais) brasileiros que avaliaram a associação entre o nível de conhecimento (Exposição) e o status vacinal (Desfecho). A qualidade metodológica foi avaliada pela escala de Newcastle-Ottawa (NOS). Uma meta-análise de efeitos aleatórios (k=2) foi realizada para calcular o Odds Ratio (OR) agregado para "não vacinação" associada ao "baixo conhecimento". Resultados: Quatro estudos transversais (k=4) foram incluídos na revisão sistemática, totalizando 2.914 participantes. A revisão sistemática revelou resultados conflitantes: dois estudos (N=1.269) encontraram uma associação estatisticamente significativa entre maior conhecimento e maior adesão vacinal, enquanto um estudo (N=1.289) não encontrou associação e um (N=492) foi puramente descritivo. A meta-análise (k=2) dos estudos que reportaram ORs ajustados resultou em um OR agregado de 1,70 (IC 95%: 0,94 – 3,06; p=0,08), indicando uma tendência não significativa. A heterogeneidade estatística foi baixa (I² = 24,1%). Conclusão: A evidência atual sobre a associação entre conhecimento e adesão à vacina contra o HPV no Brasil é limitada e conflitante. A meta-análise (k=2) é estatisticamente frágil, tem baixo poder e não permite conclusões robustas. A aparente associação positiva observada em alguns estudos individuais é contestada por outros que relatam efeito nulo. São necessários mais estudos primários robustos para determinar se intervenções educativas são suficientes para melhorar as taxas de vacinação.
