Abstract
A 64-year-old Caucasian male was referred to our hospital for a NSTEMI. The patient had undergone coronary artery bypass grafting 19 years earlier and had a mechanical mitral valve, presenting on warfarin with a therapeutic INR. His grafts included a left internal mammary artery (LIMA) to the left anterior descending (LAD) artery and a saphenous vein graft (SVG) to the second obtuse marginal (OM2) artery. Three years earlier, a routine thoracic CT angiogram at the referring hospital incidentally revealed a 41×38 mm SVG pseudoaneurysm in the proximal to mid-segment. Two months before his NSTEMI, a follow-up non-contrast thoracic CT showed an enlargement to 41×42 mm with partial thrombosis, despite long-term anticoagulation. At that time, the patient remained asymptomatic, and no intervention was deemed necessary at that time by the patient's treating physician. No further investigations or imaging were done before his presentation with a NSTEMI.
