In their comparative analysis in this issue of Cancer Cytopathology , Kowalewski et al. provide one of the first large‐scale evaluations of how the Milan and World Health Organization frameworks perform side by side in salivary gland cytology. Although affirming diagnostic performance, the results raise an important concern: whether streamlining reporting inevitably comes at the cost of clinical precision.
