Association of nocturnal intermittent hypoxemia and pulse rate in infants without or with upper airway obstruction

Abstract

Oxygen desaturation (≥3%) index(ODI3) values partially overlap in infants without respiratory disorder and those with upper airway obstruction (UAO). We hypothesized that nocturnal intermittent hypoxemia is associated with age-adjusted pulse rate in infants with UAO. <bold>Methods:</bold> Controls (1-12 mo) without respiratory disease had NOx. NOx of infants with UAO were analyzed retrospectively. <italic>Pulse rate index</italic>(PRI)=[(average pulse rate)-(95th percentile value in active sleep for specific age)] X100/(95th percentile value in active sleep) was the outcome; ODI3 and McGill oximetry score (MOS)>1 were explanatory variables. Linear regression/general linear models were used to assess potential association of ODI3 or MOS>1 with PRI and their interaction with study group regarding PRI. <bold>Results:</bold> 138 controls (median age 5.1m [range 1-12.7]) and 23 infants with UAO (2.4m [1.0-11.1]) were included. ODI3 was positively related to PRI in infants with UAO, but not in controls: beta(95%CI) 0.435(0.114-0.755);R<sup>2</sup>=0.275;<italic>P</italic><.010 vs. beta -0.127(-0.463 - 0.209);R<sup>2</sup>=0.004;P<.455. <fig><object-id>erj;66/suppl_69/PA1639/F1</object-id><object-id>F1</object-id><object-id>F1</object-id><graphic></graphic></fig> Interaction of ODI3 with group (UAO vs. controls) regarding PRI was significant (<italic>P</italic>=.023). MOS>1 or its interaction with group was not significant PRI predictor. <bold>Conclusion:</bold> Although ODI3 has lower sensitivity than MOS>1 for UAO (Eur Respir J 2024;64:PA1361), the higher the ODI3 the higher the pulse rate in infants with UAO but not in controls.
