Pharmakologische Interventionen gegen Pruritus (Juckreiz) bei Palliativpatienten

Hintergrund Pruritus ist ein belastendes Symptom, das die Lebensqualität von Patienten beeinträchtigen kann. Methode In diesem Update des Cochrane-Reviews von 2016 durchsuchten wir CENTRAL, MEDLINE und Embase im März 2021. Wir schlossen randomisierte kontrollierte Studien (RCTs) ein, welche die Wirkung von pharmakologischen Interventionen im Vergleich zu Placebo hinsichtlich des primären Endpunktes (subjektiv empfundener Juckreiz) untersuchten. Die Sicherheit der Evidenz wurde anhand der GRADE-Kriterien beurteilt. Ergebnisse Insgesamt schlossen wir 87 Studien und 4463 Teilnehmer in den Review ein (38 neue Studien und neue 2650 Patienten). Bei Patienten mit Chronic Kidney Disease-assoziiertem Pruritus (CKD-aP) führte die Behandlung mit GABA-Analoga zu einer starken Reduktion des Juckreizes (visuelle Analogskala (VAS) 0 bis 10 cm, mittlere Differenz (MD) -5,10 (95% Konfidenzintervall (KI) -5,56 bis -4,55, N=297, Sicherheit der Evidenz (SDE): hoch). Die Behandlung mit Kappa-Opioid-Rezeptor-Agonisten (Difelikefalin, Nalbuphin, Nalfurafin) reduzierte den Juckreiz leicht (MD -0,96 (-1,22 bis −0,71), N=1292, SDE: hoch). Die Behandlung mit Montelukast und Fischöl/Omega-3-Fettsäuren kann zu einer starken Verringerung des Juckreizes führen (SMD -1,40 (-1,87 bis -0,92), N=87; bzw. SMD -1,66 (-2,50 bis -0,83), N=108, SDE: gering). Die Behandlung mit Cromolyn-Natrium oder mit topischem Capsaicin führte wahrscheinlich zu einer starken Verringerung des Juckreizes (MD -3,27 (-5,91 bis -0,63), N = 100; bzw. SMD -1,06 (-1,55 bis -0.57), N=112, SDE: moderat). Ondansetron, Zinksulfat und mehrere andere Interventionen können den Juckreiz möglicherweise nicht reduzieren. Bei cholestatischem Pruritus (CP) kann die Behandlung mit Rifampicin und Flumecinol den Juckreiz reduzieren (MD -4,2 (-8,73 bis 0,31), N = 42, SDE: sehr niedrig, bzw. Risk-Ratio 2,32 (0,54-10,1), N=69, SDE: niedrig). Die Behandlung mit dem Naltrexon reduzierte wahrscheinlich den Juckreiz (MD -2,42 (-3,90 bis -0,94), N=52, SDE: moderat). Bei Teilnehmern mit Juckreiz unterschiedlicher Genese reduzierte die Behandlung mit Paroxetin wahrscheinlich den Juckreiz leicht (MD -0,78 (-1,19 bis -0,37), N=48, SDE: moderat). Schlussfolgerung GABA-Analoga, Kappa-Opioid-Rezeptor-Agonisten, Cromolyn-Natrium, Montelukast, Fischöl/Omega-3-Fettsäuren und topisches Capsaicin waren bei Chronic Kidney Disease-assoziiertem Pruritus wirksam gegen Pruritus. GABA-Analoga hatten die größte Wirkung. Rifampin, Naltrexon und Flumecinol könnten bei cholestatischem Pruritus wirksam sein. Publication History Article published online: 31 August 2022 © 2022.
