Application of Occupation-Based Practice 2.0 (OBP 2.0) in Healthcare Settings : A Case Study

Abstract
목적: 본 연구는 작업-기반 실천을 실시하기에 어려운 재활병원에 입원 중인 대상자에게 작업기능장애와 신체기능의 비 교를 통하여 작업치료 임상에서 작업-기반 실천 2.0을 적용한 유용성을 알아보고자 한다. 연구방법: 사전 인터뷰를 통하여 연구 참여에 동의한 만성 뇌졸중 환자 2명을 대상으로 하였다. 작업-기반 실천 2.0을 8 주 동안 주 2회, 회당 30분씩 실시하였고 효과를 확인하기 위해 작업기능장애와 신념대립을 평가하였다. 또한 대상자의 작업수행 문제점을 파악하기 위해 Canadian Occupational Performance Measure(COPM)을 사용하였고, 수행기능을 평가하기 위해 4가지의 평가도구를 사용하였다(Mimi-Mental State Examination; MMSE-K, Manual Function Test; MFT, Berg's Balance Scale; BBS, Modified Barthel Index; MBI). 평가기간은 초기, 중간(4주 후), 최종(8주 후)으로 나누어 진행하였다. 결과: 작업기능장애와 신념대립 평가결과는 작업-기반 실천 중재 이후 더 좋아졌다. COPM 결과는 최종평가와 초기평가 를 비교하여 두 대상자의 작업수행 수행도와 만족도를 점수화했을 때 두 대상자 모두 수행도와 만족도에서 유의한 결과 가 나타났다. 수행기능을 평가하기 위해 실시 한 4가지의 평가도구 결과는 MBI에서만 점수 상승이 나타났고, MMSE-K, MFT, BBS에서는 점수 차이가 없었다. 결론: 작업-기반 실천 2.0은 대상자의 ADL과 작업수행을 증진시키고, 작업 참여에 제한을 주는 요소들을 줄이는 임상적 으로 유용한 중재 방법으로 사용된다.
