Abstract
Contexte : Ce rapport décrit le cas d'un jeune homme atteint d'une infection à Neisseria gonorrhoeae multirésistante contractée en Ontario, au Canada, sans aucun antécédent de voyage.Méthodes : Le suivi de cas a été effectué conformément aux pratiques courantes de santé publique en Ontario.Les épreuves de sensibilité aux antimicrobiens des isolats ont été réalisés par dilution en gélose.Le typage de souche et d'autres caractérisations moléculaires ont été effectués par séquençage du génome entier.Résultats : Le patient a été traité avec succès avec de la ceftriaxone intramusculaire et de l'azithromycine par la voie orale.Les tests de dilution en gélose ont montré une sensibilité réduite à tous les agents testés, à l'exception de l'azithromycine et de la spectinomycine, y compris une non sensibilité à la ceftriaxone (concentration minimale inhibitrice [CMI] = 0,5 mg/L) et au céfixime (CMI = 2 mg/L), une résistance à la tétracycline (CMI = 2 mg/mL) et à la ciprofloxacine (CMI = 32 mg/L), et une sensibilité intermédiaire à la pénicilline (CMI = 1 mg/L).Le séquençage du génome entier a révélé que l'isolat était étroitement lié au clone FC428, qui héberge l'allèle mosaïque penA60 responsable des CMI élevées aux céphalosporines à spectre étendu, telles que la ceftriaxone ou la céfixime, toutes deux actuellement recommandées comme options de traitement de première intention ou de remplacement pour les infections de gonorrhée anogénitale sans complications en Ontario.Conclusion : L'identification de ce cas suggère qu'une transmission locale de cette souche de N. gonorrhoeae multirésistante, non reconnue auparavant, a lieu en Ontario et souligne la nécessité d'une surveillance continue afin de suivre les tendances et de guider les recommandations de traitement.
