Abstract

IgG型抗A/B抗体は,ABO不適合生体肝臓移植(肝移植)患者の移植成績に大きく影響を及ぼす.我々は,表面プラズモン共鳴(surface plasmon resonance: SPR)原理の応用により,IgG型抗A/B抗体量を迅速·簡便に測定可能なSPR法を開発した.今回,健常ボランティア血清からアフィニティーカラムクロマトグラフィーによりIgG型抗A,抗B抗体の精製を行いSPR法の検量線を作成した.この検量線よりSPR法は測定値の濃度表示化(μg/ml)が可能となった.また,25例のABO不適合肝移植患者の測定値の推移を,標準的な方法である試験管での赤血球凝集法(TT法)とSPR法,フローサイトメトリー法(FCM法)で測定した.その結果,SPR法とFCM法は高い相関が得られたが,SPR法とTT法,FCM法とTT法の相関は低い結果となった.そして,測定値の推移についてTT法,SPR法,FCM法の3法が一致した群と一致しなかった群に大別されるが,3法一致群で測定値が高値となった症例では肝臓機能検査異常や病理組織像で液性拒絶が認められ術後6カ月以内の死亡例が存在した.しかし,3法不一致群でTT法のみ高値となった症例では液性拒絶例はなく,移植後早期死亡例も認められなかった.
