Les chutes sont l’une des principales causes d’évènements indésirables survenant dans la prestation de soins et de services de santé. Dans le but de réduire leur nombre et les blessures qui en découlent, un projet d’amélioration continue de la qualité a été mis en place dans une unité de vie du centre d’hébergement de soins de longue durée (CHSLD) Hôtel-Dieu-de-Saint-Hyacinthe, au Québec, avec pour objectif une baisse de 15%. Concrètement, ce projet a permis d’implanter un nouveau processus d’analyse des incidents et accidents de type « chute » incluant les usagers, leurs proches et l’équipe interdisciplinaire, à travers l’animation quotidienne d’un caucus (comité) qualité-sécurité et la mise en place de tournées intentionnelles. En complément, l’usager et ses proches collaborent à l’identification d’actions d’amélioration personnalisées visant à éviter la récurrence des chutes. L’objectif fixé a été atteint par la diminution du taux de chutes. De plus, l’implication des résidents ou de leurs proches dans l’identification de moyens visant à éviter la récurrence d’une chute, et la valorisation des membres de l’équipe interdisciplinaire ont bonifié ce résultat. Reconnu comme pratique exemplaire visant à assurer la qualité et la sécurité des soins et services, le projet est présentement déployé dans tous les CHSLD du Centre intégré de santé et de services sociaux (CISSS) de la Montérégie-Est.
