Abstract
Η ενδομητρίωση αποτελεί μια χρόνια γυναικολογική νόσο που συνιστά σημαντικό αίτιο γυναικείας υπογονιμότητας. Η συσχέτιση της βιταμίνης D με την παθογένεση της ενδομητρίωσης υποστηρίζεται από προηγηθείσες μελέτες οι οποίες ανέδειξαν την έκφραση του υποδοχέα της βιταμίνης D (VDR) καθώς και την παρουσία της πρωτεΐνης πρόσδεσης της βιταμίνης D (VDBP) και του ενζύμου 1-α υδροξυλάση στο έκτοπο ενδομήτριο. Επιπλέον, η αντιφλεγμονώδης δράση της βιταμίνης D μπορεί να έχει ευεργετικά αποτελέσματα και να επιφέρει τη μείωση του μεγέθους των βλαβών. Παρ’όλα αυτά, τα δεδομένα που προκύπτουν από μελέτες για τον προσδιορισμό της 25-υδρόξυ βιταμίνης D σε γυναίκες με ενδομητρίωση είναι διφορούμενα. Ο σκοπός αυτής της συστηματικής ανασκόπησης είναι ο προσδιορισμός του επιπολασμού της έλλειψης ή ανεπάρκειας της βιταμίνης D σε γυναίκες με ενδομητρίωση σε σχέση με υγιείς γυναίκες. Στην αναζήτηση συμπεριλήφθηκαν μεγάλες βάσεις δεδομένων (PubMed, Scopus, Cochrane Database of Systematic Reviews και Web of Science) καθώς και γκρίζα βιβλιογραφία. Για την ποιοτική αξιολόγηση των αποτελεσμάτων χρησιμοποιήθηκε η κλίμακα Newcastle-Ottawa. Μελέτες με 8-9 βαθμούς αξιολογήθηκαν ως υψηλής ποιότητας ενώ μελέτες με 6-7 βαθμούς ως μέτριας ποιότητας. Aπό 511 μελέτες, οκτώ μελέτες αξιολογήθηκαν πλήρως και τέσσερις πληρούσαν τα κριτήρια εισόδου και συμπεριλήφθηκαν στην συστηματική ανασκόπηση. Μία μελέτη κοόρτης έλαβε 8 βαθμούς, δύο μελέτες ασθενών-μαρτύρων 6 βαθμούς και άλλη μία 4 βαθμούς. Τα αποτελέσματα των δύο μελετών καλύτερης ποιότητας συνηγορούσαν υπέρ συσχέτισης της ενδομητρίωσης με χαμηλές τιμές 25-υδρόξυ-βιταμίνης D, όμως, οι άλλες δύο μελέτες παρουσίασαν αντικρουόμενα αποτελέσματα. Συμπερασματικά, απαιτούνται περισσότερες και μεγαλύτερες μελέτες κοόρτης για την επαλήθευση των ευρημάτων
