The association between anxiety and depression symptoms and chronic pain characteristics in patients with hip and knee osteoarthritis

Resumo Objetivo. Este estudo teve como objetivo estimar a prevalência de sintomas de ansiedade e depressão em doentes com dor crónica (DC) devido a osteoartrose (OA) e explorar a associação destes sintomas com as características clínicas da dor. Métodos. Neste estudo transversal, foram incluídos doentes adultos com DC e OA do joelho e/ou anca. Os sintomas de ansiedade e depressão foram avaliados utilizando a Escala de Ansiedade e Depressão Hospitalar. A escala visual analógica, o Western Ontario and McMaster Universities Arthritis Index e o PainDetect Questionnaire foram utilizados para avaliar as características da dor e o Health Assessment Questionnaire para avaliar a qualidade de vida relacionada com a saúde (QVRS). Os coeficientes de correlação foram utilizados para explorar as associações entre os sintomas de ansiedade e depressão e as características da dor e a QVRS. Resultados. Foram incluídos 61 doentes (66.2±9.4 anos, 67.2% do sexo feminino). A maioria (70.5%) dos doentes apresentava sintomas de ansiedade e/ou depressão clinicamente significativos. Os doentes com sintomas de ansiedade e/ou depressão apresentavam maior intensidade da dor e maior incapacidade física. Verificou-se uma correlação positiva fraca com a dor (r=0.358) e a rigidez (r=0.364) e uma correlação positiva moderada com a função física (r=0.520) e a QVRS (r=0.405). Conclusão. Os sintomas de ansiedade e depressão são comuns em doentes com OA e DC e foram associados a uma maior intensidade da dor, maior incapacidade física e menor QVRS. Estes resultados demonstram a importância do rastreio de sintomas de ansiedade e depressão em doentes com OA, de forma a desenvolver tratamentos multidisciplinares mais eficazes.
