Introduction: In Québec, an upgraded booster seat law (UBSL) was implemented on April 18, 2019, requiring children under the age of 9 or shorter than 145 cm to use child restraints including a booster seat. Objectives: This study aims i) to examine the relationship between UBSL and child restraint system usage at crash (CRS-UC) and ii) to evaluate changes in child injury outcomes in Québec before and after the UBSL. Methods: Data from the Société de l’assurance automobile du Québec and Statistics Canada covering the period from 2016 to 2022 were utilized. An interrupted time series analysis was conducted to examine changes in three key monthly rates: CRS-UC, minor injuries, and serious or fatal injuries (SFI) before and after the policy implementation. These outcomes were analyzed using two denominators: per 100,000 children aged 6 to 9 years and per 1,000,000 registered vehicles. The analysis controlled for four covariates: time trends, seasonality, presence of the COVID-19 pandemic, and unemployment rate. Results: The study found that, in the unadjusted analysis, the mean CRS-UC rate increased by approximately 18% following the implementation of UBSL. After adjusting for covariates, CRS-UC per population showed an increase of 39% (incidence rate ratio [IRR]: 1.39; 95% CI: 1.08–1.79) (similar results observed per vehicles). Unadjusted analyses indicated a 36% reduction in mean minor injury rates, while SFI rates remained unchanged. After adjusting for covariates, no association, per population, was statistically significant at the 0.05 level between UBSL and minor injury rates (IRR: 1.02; 95% CI: 0.83– 1.25) or SFI rates (IRR: 0.37; 95% CI: 0.09–1.39) (similar results per vehicles). Conclusion: This study is the first to evaluate the relationship between UBSL in Québec and the child passenger safety indicators. The lack of a significant association between the UBSL and injury rates could suggest that the upgrade of booster seat legislation to UBSL is not an effective strategy to reduce the risk or that the complementary actions (e.g., among other measures, public education campaigns aimed at reducing in misuse of CRS) may be necessary to enhance the potential effects of child passenger safety policies like the UBSL.
