Abstract
Objectifs : Déterminer si la cure thermale, associée aux exercices à domicile et au traitement médical habituel, présente un avantage par rapport aux exercices et au traitement habituel dans le traitement de l’arthrose du genou. Méthodologie : essai clinique prospectif randomisé et multicentrique de forte puissance Participants : patients souffrant d’arthrose du genou selon les critères de l’American College of Rheumatology, et réalisant une cure thermale en ambulatoire en France. Traitement : L’ensemble des patients poursuivaient leurs traitements habituels et réalisaient quotidiennement des exercices standardisés à domicile. Le groupe thermal bénéficiait en plus d’une cure thermale de 18 jours (avec massages sous l’eau, douches, boues, baignoires à hydrojet, mobilisation et hydromassages en piscine). Critère de jugement principal : nombre de patients atteignant une amélioration cliniquement importante à 6 mois, défini comme ≥19.9 mm sur l’échelle visuelle analogique pour la douleur et/ou ≥9.1 points sur l’indice Western Ontario and McMaster Osteoarthritis Index et absence de chirurgie du genou. Randomisation : par blocs avec insu partiel du patient selon la méthode Insu : Le personnel de l’établissement thermal ne savait pas quels patients participaient à l’étude. Résultats : 630 patients ont été examinés entre juin 2006 et avril 2007. 462 patients ont été randomisés, l’analyse en intention de traiter a porté sur 187 patients dans le groupe témoin et 195 dans le groupe cure thermale. A 6 mois, 99/195 (50,8%) des patients du groupe cure thermale ont eu une amélioration cliniquement pertinente et 66/187 (36,4%) dans le groupe contrôle (χ² = 8.05 ; df = 1 ; p = 0.005). Il n’y avait pas de différence entre les deux groupes pour la qualité de vie (SF36) ni dans le fait de se trouver dans un état cliniquement acceptable pour les patients. Conclusion : Pour les patients souffrant d’arthrose du genou, une cure thermale de 3 semaines, accompagnée d’exercices à domicile et des traitements médicamenteux habituels est supérieure, à 6 mois aux exercices à domicile et à la poursuite du traitement habituel. Elle est bien tolérée.
