Abstract

Amaç: AS hastalık tanısı almış kişilerde fiziksel aktivite düzeyini ve etkileyen faktörleri (kinezyofobi, hastalık aktivitesi, fonksiyonel durum, ağrı) incelemektir. 
\nGereç ve Yöntem: Çalışmaya, İstanbul Üniversitesi Cerrahpaşa Tıp Fakültesi Hastanesi romatoloji kliniğine başvuran, ortalama yaşları 32,6 (± 5,041) yıl olan 30 ankilozan spondilit tanısı konmuş olgular ve 30 sağlıklı kontroller dahil edildi. Olguların fiziksel altivite düzeyi Uluslararası Fiziksel Aktivite Anketi (IPAQ) Uzun Formu ve akselerometre ile değerlendirildi. Kinezyofobi değerleri TAMPA Kinezyofobi Ölçeği ile, ağrıları Kısa form McGill Ağrı Anketi ile, hastalık aktiviteleri Bath Ankilozan Spondilit Hastalık Aktivitesi İndeksi (BASDAİ) ile, fonksiyonellikleri Bath Ankilozan Spondilit Fonksiyonel İndeksi (BASFİ) ile değerlendirildi.
\nBulgular: Yapılan değerlendirmeler sonucunda kontrol ve deney grupları arasında IPAQ’a göre total fiziksel aktivite skorları arasında istatistiksel olarak anlamlı bir fark saptanmadı (p>0,05). Olguların akselerometre sonuçlarına göre dakika cinsinden toplam düşük, orta ve şiddetli fiziksel aktivite düzeyleri arasında istatistiksel olarak anlamlı bir farklılık saptanmadı (p>0,05). Kontrol ve deney grupları arasında akselerometre sonuçlarına göre şiddetli fiziksel aktivite düzeyi ile hastalık aktivitesi arasında negatif bir korelasyon saptandı (p<0,05). BASDAİ total skoru arttıkça şiddetli fiziksel aktivite düzeyi azaldığı görüldü. Kontrol ve deney grupları arasında akselerometre sonuçlarına göre orta ve şiddetli fiziksel aktivite düzeyi ile fonksiyonel durum arasında negatif bir korelasyon saptandı (p<0,05). Orta ve şiddetli fiziksel aktivite düzeyi artıkça BASFİ total skoru azaldığı görüldü. 
\nSonuçlar: AS tanılı hasta populasyonunda ileri dönemlerde postüral ve fonksiyonel kısıtlılıkların şiddetinin azaltılması için rehabilitasyon programları daha erken yaşlarda planlanmalı ve hastalar egzersiz yapmak için teşvik edilmelidir.

\nABSTRACT
\nPurpose: Our aim was to investigate physical activity level and the factors affecting the level of physical activity (kinesiophobia, pain, disease activity, functional status) in patients who were diagnosed with ankylosing spondylitis (AS).
\nMethod: Thirty participant who diagnosed AS, with a mean age 32,6 (±5,041) years who consulted to İstanbul University Cerrahpaşa Faculty of Medicine Hospital and thirty healthy controls were recruited to this study. Physical activity level was evaluated with International Physical Activity Questionnaire (IPAQ) and accelerometer. Tampa Kinesiophobia index was used to measure kinesiophobia scores. Pain was evaluated with McGill Pain Questionnaire, disease activity was evaluated with Bath Ankylosing Spondylitis Disease Activity Index, functional status was evaluated with Bath Ankylosing Spondylitis Functional Index. 
\nResults: After the evaluations, we found that there was no statistically significant difference between control and patient groups according to IPAQ (p>0,05). Besides, there was no statistically significant difference between low, moderate, vigorous physical activity levels according to accelerometer in between the control and patients groups (p>0,05). However, according to accelerometer there was a statistically significant correlations (p<0,05) between vigorous physical activity levels and BASDAI total scores. So, when BASDAI total score increased, vigorous physical activity level decreased. At the same time, according to accelerometer there was a significant correlation (p<0,05) between vigorous and moderate physical activity levels and BASFİ total scores. So, when vigorous and moderate physical activity levels increased, BASFİ total score decreased. 
\nConclusions: Patients should be encouraged for exercise. Rehabilitation programmes should be planned in early ages for reduce the postural and functional impairments in advanced stages in population with AS.
