Abstract
Seututerveyskeskus tarjoaa perusterveydenhuollon palveluja 19 000 kuntalaiselle Laukaassa. Seututerveyskeskuksen strategiassa painotetaan asukkaiden osallistumista omaan hoitoonsa ja että toiminta on tutkitun tiedon mukaista (Strategia 2021). Seututerveyskeskuksen laatutaulun tavoitteissa on hoitosuunnitelman käyttö. \n \nTerveyshyötymalli tuo asiakaskeskeisen näkökulman terveydenhuoltoon. Terveys- ja hoitosuunnitelma on osa terveyshyötymallia ja sen keskiössä asiakkaan omat tarpeet ja tavoitteet. Terveys- ja hoitosuunnitelman käyttö säästää resursseja ja lisää asiakkaan saamia terveyshyötyjä. \n \nTutkimuksellisen kehittämistyön tavoitteena oli, että Laukaan terveyskeskuksen avovastaanoton pitkäaikaissairaat saavat näyttöön perustuvaa, ennaltaehkäisevää ja yksilöllistä hoitoa, joka perustuu asiakkaan ja ammattilaisen yhteistyössä tekemään suunnitelmaan. Opinnäytetyön tavoitteena oli saada hoitosuunnitelman teko implementoitua terveysaseman vastaanotolle vertaiskehittämisen avulla. Opinnäytetyön kehittämistehtävänä oli selvittää implementoitumisen estävät ja edistävät tekijät Laukaan terveyskeskuksen hoitajien vastaanotoilla. \n \nImplementoinnin prosessissa käytettiin Ottawa-mallia ja Implementointitutkimuksen viitekehystä. Prosessin alussa selvitettiin implementoinnin estävät ja edistävät tekijät. Tämän jälkeen toteutettiin implementointi, jonka lopetus ja arviointi vaiheessa teemahaastattelulla katsottiin, onko ammattilaisen työskentelyssä muutoksia. \n \nTyön aikana saatiin tuloksia terveys- ja hoitosuunnitelman implementoitumiseen vaikuttavista tekijöistä Laukaan terveyskeskuksen hoitajien vastaanotoilta. Tulokset olivat yhteneväisiä teoriatiedon kanssa. Uusien työnteonmallien jalkauttaminen vaatii koko organisaation ja johdon tuen taakseen. Yhteiset käytännöt ja säännöt ovat keskeisessä osassa. Koulutus todettiin tuloksissa myös tärkeäksi implementoitumisen tekijäksi. Organisaation implementoinnin prosessin huomioiminen nousi Kehittämisehdotukseksi.
