O alo-HSCT é considerado uma abordagem terapêutica altamente curativa para doentes com leucemias agudas. No entanto, sendo a recidiva e consequente progressão da doença a principal complicação, é necessário implementar estratégias de monitorização que sejam eficazes na sua antecipação. A avaliação do quimerismo, fornece informação acerca da efetivação/rejeição do enxerto, funcionando como excelente fator de prognóstico. Verificar a importância do estudo das células CD34+, no follow-up do alo-HSCT nas leucemias agudas face a outros marcadores. Revisão sistemática de acordo com as diretrizes PRISMA. Recolha dos artigos efetuada em diferentes bases de dados, inclusão de estudos de coorte, caso-controlo e ensaio clínico randomizado, sem limite temporal de publicação. Análise metodológica avaliada pelas Escalas Newcastle Ottawa e Jadad. Da totalidade de 440 artigos estudados, foram incluídos 16. A maioria dos estudos obtiveram resultados pertinentes, na análise do quimerismo das células CD34+, visto que, este foi altamente preditivo de recidiva, face aos outros marcadores. A análise do quimerismo das células CD34+ apresentou maior sensibilidade face ao quimerismo convencional, na deteção de recidiva. Mostrou ser uma abordagem viável e sensível na avaliação da eficácia do transplante e prognóstico da patologia, permitindo monitorizar a recidiva após alo-HSCT.
