Epävakaiden jalkineiden vaikutuksia polven nivelrikkoa sairastavien kokemaan kipuun ja toimintakykyyn

Abstract
Opinnäytetyönä tekemäämme tutkimuksen tavoitteena oli selvittää voiko epävakaiden jalkineiden 7 viikon käyttö vähentää koettua nivelen kipua, niveljäykkyyttä sekä edistää fyysistä toimintakykyä polven nivelrikkoa sairastavilta. \n \nKoeryhmään kuului 8 naista ja 2 miestä, joiden ikäkeskiarvo oli 58 vuotta. Koehenkilöistä kolmella (3) oli nivelrikon aiheuttamia kipuja oikeassa polvessa ja seitsemällä (7) molemmissa. He täyttivät ennen 7 viikon käyttöjaksoa sekä sen jälkeen WOMAC(Western Ontario and McMaster Universities Osteoarthritis index) kyselylomakkeen. Kyselylomakkeessa arvioidaan subjektiivista kipua, niveljäykkyyttä ja koettua toimintakykyä. Lisäksi heiltä mitattiin nivelliikkuvuudet polvi- ja lonkkanivelen osalta sekä testattiin toimintakykyä 10 metrin kävelytestillä, tuolilta ylösnousu testillä, timed up and go testillä sekä jalan vuoroittainen nosto porrasaskelmalle testillä. Tässä tutkimuksessa käytettiin Masai Barefoot Technology (MBT) jalkineita. \n \nWOMAC kyselylomakkeella mitattu nivelkipu sekä koettu toimintakyvyn puute vähenivät ryhmässä merkitsevästi (p<0,01) ja niveljäykkyys väheni melkein merkitsevästi intervention aikana (p<0,05). Nivelkipu väheni 55,8 %, niveljäykkyys väheni 46,9 % ja koettu toimintakyvyn puute väheni 58 %. Itse valitulla kävelynopeudella koeryhmän kävelyajassa oli parannusta 6,8 % (p<0,05) ja maksimi kävelynopeudella kävelyajassa oli parannusta 8,5 % (p<0,05). Timed Up and Go testissä parannusta oli 9,8 % (p<0,05). Tuolilta ylösnousussa ryhmän keskiarvo parani 21,6 % (p<0,01). Vuorottainen jalan nosto askelmalle parani 22,2 % (p<0,001). Nivelliikkuvuusmittauksissa merkitseviä tuloksia antoivat oikean lonkkanivelen passiivinen (p=0,008) ja aktiivinen (p=0,008) adduktio sekä vasemmalla merkittäviä tuloksia oli lonkkanivelenaktiivisessa ulkorotaatiossa (p=0,008) ja polvinivelen aktiivisessa fleksiosta (p=0,004). \n \nEpävakaiden jalkineiden 7 viikon käyttö vähensi koettua nivelen kipua, niveljäykkyyttä sekä paransi toimintakykyä WOMAC kyselylomakkeella ja toimintakyvyn testeillä mitattuna.
