L'utilisation d'un bilan pour investiguer l'étiologie du délirium est une pratique courante dans le domaine médical, mais qui repose sur peu d'évidence. L'objectif de ce projet est de déterminer la pratique actuelle des médecins canadiens concernant l'investigation du patient en délirium, en portant une attention spécifique sur l'investigation et la prise en charge de la bactériurie asymptomatique chez le patient en délirium. Notre hypothèse soutient que la pratique actuelle des médecins canadiens est hétérogène en ce qui a trait aux investigations du patient en délirium et que ceux-ci demeurent très interventionnistes pour l'investigation et le traitement de la bactériurie asymptomatique. Un sondage en ligne a été construit puis distribué via diverses associations médicales canadiennes. Cette étude transversale ciblait les médecins canadiens qui prennent en charge des patients en délirium dans le cadre de leur pratique. Un total de 297 médecins a été inclus. Le bilan initial d'investigations toujours demandé par plus de 80% des répondants comprend la formule sanguine, la créatinine, et les électrolytes. Les autres tests étaient demandés soit à l'occasion ou jamais. De plus, 79,4% de nos participants demandent une bandelette urinaire ou une analyse d'urine chez les patients en délirium. En cas de découverte d'une bactériurie chez ces patients afébriles sans symptômes urinaires, 38% des médecins traitent immédiatement et seulement 13,7% s'abstiennent de prescrire des antibiotiques. Finalement, 92,5% des participants croient que des lignes directrices claires sont nécessaires pour guider le bilan d'investigations. La pratique des médecins pour l'investigation des patients en délirium est grandement variable telle que présumée. L'étude démontre la prise en charge hétérogène de la bactériurie asymptomatique chez les patients en délirium et la nécessité de lignes directrices claires.
