Abstract
"Introdução: O diagnóstico da fragilidade é complexo e difícil. O médico de Medicina Geral e Familiar desempenha um papel fundamental na identificação precoce dos idosos frágeis. Objetivo: Clarificar e propor à comunidade clínica em Portugal o conceito de fragilidade nos cuidados de saúde primários e continuadas para que se adapte e valide ferramentas de diagnósticos sensíveis, simples e rápidas, a serem aplicadas por qualquer profissional de saúde. Metodologia: Na pesquisa efetuada as escalas de fragilidade com maior realce foram: Fenótipo de Fragilidade de Fried, 2001 (ferramenta utilizada para classificar os idosos segundo o seu perfil de fragilidade e os cinco itens que o compõem, a redução da força de preensão palmar, o nível de atividade física e a lentidão da marcha podem sofrer influência de características antropométricas da população) e a escala clínica de fragilidade ou clinical frailty scale de K. Rockwood, 2004 (escala com um score de 9 itens que se baseia no canadian study of health and aging (CSHA) e considera que a condição de fragilidade está associada a um conjunto de défices cumulativos (físicos e psicossociais) que colocam em causa a homeostase do sujeito e ameaçam a independência do mesmo. Resultados: Este estudo permitiu identificar cerca de 15 a 25% de indivíduos com mais de 65 anos e do grupo etário com mais de 85 anos estão identificados como idosos frágeis. Conclusão: Prevenir a síndrome de fragilidade é uma prioridade para que a população idosa tenha uma melhor qualidade de vida, cabendo aos profissionais de saúde esta função."
