"Objetivo: analizar la utilidad del empleo de las reglas de Ottawa para el diagnóstico de lesio- nes del pie o tobillo. Material y métodos: estudio exploratorio efec- tuado en el servicio de Urgencias del Hospital de Traumatología “Magdalena de las Salinas”. Participaron adultos con lesiones del pie o to- billo atendidos durante enero de 2003; la mues- tra fue no probabilística por conveniencia. A los pacientes que acudieron para su atención por lesiones del tobillo o pie, sin deformidad evidente, se les realizó serie radiográfica y se les v aloró mediante los siguientes criterios: a) dolor o aumento en la sensibilidad en los 6 cm distales del borde posterior de la tibia y el peroné hasta la punta del maléolo lateral o medial; b) dolor o aumento en la sensibilidad del escafoides o en la base del quinto metat arsiano; c) incapacidad para soport ar el pe so corporal inmediatamente después de ocurrida la lesión y durante la exploración física. La pres encia de uno o más criterios indicó la necesidad de reali- zar serie radiográfica. Se estimó sensibilidad, especificidad, valores predictivos y tasas de probabilidad. Resultados: se estudiaron 50 tobillos, correla- cionando la prueba clínica con las radiogra- fías. La prueba clínica mostró sen sibilidad de 100 %, especificidad de 60 %, valor predictivo positivo de 30 %, valor predictivo negativo de 100 %, tasa de falsos positivos de 40 %, tasa de falsos negativos de 0, tasa de probabilidad cuando la prueba es positiva de 3, y cuando es negativa de 0. Conclusiones: cuando las reglas de Ottawa son negativas no está indicada la serie radiográfica, pues la probabilidad de que no exista fractura puede ser de 100 %. Si se solicita serie radio- gráfica cuando se cumple uno o más criterios, sólo un tercio de los pacientes tendrá fractura. Al parecer en las lesiones del tobillo y pie pue- de prescindirse de ciertos estudios sin dete- rioro de la calidad de la atención médica."
