Verenkiertovajauspotilaiden aivojen happeutumisen yhteys kognitiiviseen tasoon tehohoidon päättyessä : pilottitutkimus

Abstract
Tavoitteet: Tehohoidosta selvinneillä potilailla on suuri riski pitkäaikaisiin kognitiivisiin vaikeuksiin, mutta niiden taustalla olevista syistä tiedetään vasta vähän. Tutkimustiedon perusteella on viitteitä siitä, että tehohoidon jälkeisen kognitiivisen heikentymisen taustalla voisi olla aivojen heikko happeutuminen tehohoidon aikana, mutta tätä ei ole aiemmin juurikaan tutkittu. Tavoitteena oli selvittää, onko tehohoidon aikainen aivojen heikko happeutuminen yhteydessä kognitiiviseen heikentymiseen tehohoidon päättyessä ja näyttääkö ikä olevan riskitekijä. Menetelmät: Tutkittavina oli kahdeksan (8) 25–74-vuotiasta verenkiertovajauspotilasta, joista puolet oli naisia ja puolet miehiä. Aivojen happisaturaatiota mitattiin tehohoidon aikana lähi-infrapunaspektroskopialla (NIRS) ja kognitiivinen arviointi tehtiin teho-osastolta pois siirryttäessä Montreal Cognitive Assessment -testillä. Aivojen happeutumisen ja kognition välistä yhteyttä tarkasteltiin regressioanalyysillä. Tarkastelun kohteena oli happeutumisen itsenäinen yhteys kognitioon sekä yhteys, kun potilaan iän ja sukupuolen vaikutus oli kontrolloitu. Tulokset ja johtopäätökset: Tehohoidon aikainen aivojen happeutuminen oli viitteellisesti yhteydessä tehohoidon jälkeiseen kognitioon siten, että kun happisaturaatio oli ollut matalampi, kognitiivinen suoriutuminen oli heikompaa. Tällä aineistolla yhteys ei ollut tilastollisesti merkitsevä, ja kun iän sekä sukupuolen vaikutus kontrolloitiin, ei yhteyttä ollut enää havaittavissa. Iäkkäämmillä potilailla tehohoidon aikainen happisaturaatio oli matalampi ja tehohoidon jälkeinen kognitiivinen suoriutuminen heikompaa. Lisää tutkimusta tarvitaan isommalla aineistolla ja iän vaikutus kontrolloiden.
