Enquadramento: A dor lombar (DL) continua a ser a principal causa mundial de incapacidade, associada a um elevado consumo de recursos de saúde. Cerca de 2 a 10% dos utentes com episódio de DL desenvolverão dor lombar crónica (DLC). O exercício é uma das modalidades mais utilizadas e é recomendado pela literatura atual no tratamento da DLC. Apesar de efetivo, os estudos que abordam o exercício não apresentam detalhes das intervenções testadas, como são executadas e estruturadas, não permitindo identificar qual a melhor intervenção para esta população. Objetivo: Comparar os efeitos de um programa de exercício estruturado versus a Fisioterapia multimodal em utentes com DLC não específica (DLCne) ao nível da incapacidade funcional, da perceção global de melhoria, da cinesiofobia e da autoeficácia. Metodologia: Trata-se de um estudo quase-experimental com 30 indivíduos com DLC. Os participantes do grupo de exercício estruturado (n=15), realizaram o programa Rehmove de 6 semanas constituído por 12 sessões. No grupo de Fisioterapia multimodal (n=15), realizaram um programa de 6 semanas com uma variabilidade de procedimentos utilizados. Os participantes foram avaliados antes da intervenção (baseline), após 2 semanas, após intervenção (6 semanas) e follow-up após 3 meses. Os instrumentos utilizados foram a Quebec Back Pain Disability Scale (QBPDS-PT), a Global Perceived Effect Scale (GPES-PT), a Tampa Scale of Kinesiophobia-13 (TSK-13-PT) e a Chronic Pain Self-Efficacy Scale (CPSS-PT). Avaliou-se também na baseline, o risco de dor persistente através da Start Back Screening Tool (SBST). Resultados: Os outcomes mensurados, entre os grupos não verificaram diferenças estatisticamente significativas, mas na análise intragrupos, o grupo de Exercício Estruturado (GEE) apresentou melhorias a nível da cinesiofobia e da perceção global de melhoria. Ainda sobre a análise intragrupos, o grupo de Fisioterapia Multimodal (GFM), apresentou diferenças estatisticamente significativas na autoeficácia na subcategoria funcionalidade. Relativamente à incapacidade funcional, verificou-se diferenças estatisticamente significativas no GEE, exceto em um momento avaliativo, e no GFM em todos os momentos avaliativos. Conclusão: Os resultados não mostram que um Programa de Exercício Estruturado seja mais efetivo que a Fisioterapia Multimodal nos outcomes em estudo, mas em ambos os grupos se verificou um efeito estatisticamente significativo na incapacidade funcional.
