Objektif: Belum ada penelitian yang dapat menjadi dasar pemilihan terapi penunjang pengobatan antidepresan dalam memperbaiki fungsi kognitif penderita depresi. Tujuan dari penelitian ini adalah membandingkan efektivitas penambahan terapi repetitive transcranial magnetic stimulation (rTMS) dan latihan aerobic terhadap fungsi kognitif penderita depresi.Metode: Penelitian randomized-controlled trial tiga kelompok dilakukan antara Mei 2019 hingga Januari 2020. Pasien depresi (usia 21-64 tahun, episode pertama), dengan depresi tingkat sedang dan berat, direkrut. dan dialokasikan secara acak ke dalam tiga kelompok. Dua puluh tujuh orang subjek dibagi dalam tiga kelompok, yaitu kelompok kontrol yang hanya mendapat pengobatan antidepresan, kelompok intervensi terapi rTMS yang mendapat penambahan terapi rTMS, serta kelompok intervensi latihan aerobik yang mendapat penambahan latihan aerobik. Setiap subjek pada ketiga kelompok mendapatkan pengobatan antidepresan golongan selective serotonine reuptake inhibitor (SSRI) Penilaian Montreal Cognitive Assessment (MOCA) diperiksa sebelum sesi intervensi pertama dan segera setelah sesi intervensi terakhir pada minggu ke-2. Hasil: Pada kelompok kontrol, nilai MOCA meningkat secara signifikan dari 23.78±3.032 menjadi 26.56±1.333 (p=0.007). Pada kelompok intervensi terapi rTMS, nilai MOCA meningkat secara signifikan dari 23.75±1.909 menjadi 27.13±0.354 (p=0.011). Pada kelompok intervensi latihan aerobik, nilai MOCA meningkat secara signifikan dari 23.43±1.272 menjadi 27.29±0.488 (p=0.017). Terdapat perbedaan perubahan nilai subtes delayed recall (p=0.01). Analisa post hoc memperlihatkan perbedaan signifikan perubahan nilai subtes delayed recall pada kelompok intervensi rTMS dibandingkan kelompok kontrol (p=0.005). Kesimpulan: Studi ini menunjukkan bahwa penambahan terapi rTMS pada pengobatan antidepresan menghasilkan peningkatan fungsi kognitif yang lebih baik.
