Validação da Bateria de Avaliação Frontal para a População Portuguesa em Apoio Institucional com Idade ≥ 60 Anos

Abstract
Contexto: A Bateria de Avaliação Frontal (FAB) é uma bateria rápida e de fácil aplicação, que avalia o funcionamento executivo frontal. Dado que o envelhecimento, normal ou patológico, está associado a alterações no lobo frontal, que poderão conduzir a disfunções executivas, torna-se pertinente a validação da FAB para uma população em apoio institucional com idade ≥ 60 anos. Objetivo: Validar a FAB, a partir da análise da sua utilidade clínica e das suas propriedades psicométricas, para pessoas em apoio institucional com idade ≥ 60 anos. Método: Incluímos 635 sujeitos em apoio institucional com idade ≥ 60 anos (165 homens / 470 mulheres) com idades entre os 60 e os 100 anos (M = 80,86 ± 7,62). Todos os sujeitos foram avaliados com a FAB, a Avaliação Cognitiva de Montreal (MoCA), o Mini-Exame do Estado Mental (MMSE), a Escala de Funcionalidade Geriátrica (EFG), a Escala de Depressão Geriátrica (GDS) e o Inventário de Ansiedade Geriátrica (GAI). Resultados: A FAB revelou uma consistência interna adequada (α de Cronbach = 0,77) e com correlações item-total entre moderadas a elevadas em quatro das seis subescalas. A validade convergente foi suportada por correlações moderadas entre a FAB e restantes instrumentos de avaliação neuropsicológica (MMSE, MoCA, EFG). As pontuações da FAB variaram consoante o sexo, idade, escolaridade, profissão e área de residência. A precisão diagnóstica revelou-se adequada e foi determinado um ponto de corte 6 para detetar disfunção executiva numa amostra em apoio institucional com idade ≥ 60 anos. As cinco subescalas da FAB apresentaram poder discriminativo para diferenciar entre a presença e ausência de alterações nas funções executivas. Conclusão: A FAB é um instrumento válido e fiável para avaliar disfunções frontais numa amostra de pessoas em apoio institucional com idade ≥ 60 anos. Devido à facilidade da sua aplicação e a sua precisão para detetar a presença ou ausência de disfunções executivas, é possível intervir numa amostra de pessoas em apoio institucional com idade ≥ 60 anos, com vista a melhorar o funcionamento executivo e retardar possíveis declínios a nível executivo e suas consequências. / Background: The Frontal Assessment Battery (FAB) is a quick and easy to apply battery that assesses frontal executive functioning. Given that ageing, normal or pathological, is associated with changes in the frontal lobe, which may lead to executive dysfunction, the validation of the FAB for a population in institutional support aged ≥ 60 years becomes important. Purpose: Validate the Frontal Assessment Battery by analyzing its clinical utility and its psychometric properties for the Portuguese population in institutional support aged ≥ 60 years. Method: Six hundred and thirty-five institutionalized subjects (165 men / 470 women) aged 60 to 100 years (M = 80.86 ± 7.62) were assessed with the Frontal Assessment Battery (FAB), the Montreal Cognitive Assessment (MoCA), the Mini-Mental State Examination (MMSE), the Geriatric Functional Rating Scale (EFG), the Geriatric Depression Scale (GDS), and the Geriatric Anxiety Inventory (GAI). Results: The FAB showed adequate internal consistency (Cronbach's α = .77) and moderate to high item-total correlations in four subscales. Convergent validity was supported by a moderate correlation between the FAB and the Neuropsychological Assessment instruments (MMSE, MoCA, and EFG). FAB scores varied by gender, age, education, and area of residence. The diagnostic accuracy proved to be adequate, and a cut-off point of 6 to detect executive dysfunction in a sample in institutional support aged ≥ 60 years. The five FAB subscales showed discriminative power in differentiating between the presence and absence of brain function deficit. Conclusion: FAB is a valid and reliable instrument for assessing frontal dysfunction in a sample in institutional settings aged ≥ 60 years. Due to its easy application and its accuracy in detecting the presence or absence of executive dysfunction, it is possible to facilitate the intervention in the population in institutional support aged ≥ 60 years to improve executive functioning and delay possible declines at the executive level.
