Comparar la proporción de colelitiasis entre pacientes
\nsometidos a gastrectomía en manga (GM) y bypass gástrico en Y de
\nRoux (BGYR) mediante una revisión sistemática y metaanálisis.
\nMaterial y métodos: Se realizó una búsqueda en PubMed, Scopus,
\nWeb of Science y EMBASE siguiendo la metodología PRISMA 2020. Se
\nincluyeron estudios observacionales (cohortes y casos y controles) que
\nevaluaron la incidencia de colelitiasis tras GM y BGYR. La evaluación
\ndel riesgo de sesgo se realizó con la escala de Newcastle-Ottawa (NOS).
\nSe calculó el odds ratio (OR) combinado con intervalos de confianza al
\n95% utilizando un modelo de efectos aleatorios.
\nResultados: De 2,104 registros identificados, se incluyeron 10 estudios
\n(n = 195,665 pacientes). La mayoría de los estudios mostró un riesgo
\nreducido de colelitiasis tras GM en comparación con BGYR. El OR
\ncombinado fue 0.68 (IC 95%: 0.53–0.88, p = 0.003), indicando un menor
\nriesgo de colelitiasis en la GM. Sin embargo, se observó alta
\nheterogeneidad entre los estudios (I² = 75%), posiblemente atribuible a
\ndiferencias en diseño, características poblacionales y métodos de
\nseguimiento. La calidad metodológica de los estudios fue moderada a
\nalta, con puntuaciones NOS entre 6 y 8.
\nConclusiones: La gastrectomía en manga se asocia a una menor
\nincidencia de colelitiasis en comparación con el bypass gástrico en Y de
\nRoux. Sin embargo, la heterogeneidad observada resalta la necesidad
\nde estudios adicionales con mayor uniformidad metodológica
