Objectif : Étudier l’efficacité d’une nouvelle molécule anti-VEGF appelée brolucizumab-BEOVU© dans le cadre du traitement de la DMLA exsudative par comparaison avec la molécule anti-VEGF aflibercept- EYLEA©. Nous exprimons l’hypothèse que le traitement par IVT anti-VEGF brolucizumab-BEOVU© est un traitement prometteur révélant de meilleurs résultats visuels et anatomiques que le traitement par IVT anti-VEGF aflibercept-EYLEA© chez les patients atteints de DMLA exsudative. Méthodes : Nous avons comparé l’efficacité du traitement par IVT anti-VEGF brolucizumab-BEOVU© et aflibercept-EYLEA© à l’aide de l’échelle ETDRS et l’OCT NIDEK RS-3000 chez 60 patients atteints de DMLA exsudative dont 30 patients injectés par la molécule aflibercept-EYLEA© et 30 patients injectés par brolucizumab-BEOVU© pendant 20 semaines. Les critères d’évaluation sont l’acuité visuelle, l’épaisseur maculaire centrale, la présence ou l’absence de liquide dans la rétine de l’oeil injecté et le nombre d’IVT anti-VEGF entre la semaine 12 et la semaine 20. Résultats : Le traitement par IVT anti-VEGF brolucizumab-BEOVU© a démontré sa non-infériorité par rapport à l’IVT anti-VEGF aflibercept-EYLEA© chez les patients atteints de DMLA exsudative. Il n’existe aucune différence de gain d’acuité visuelle entre les deux groupes au cours des 20 semaines de traitement. La diminution de l’épaisseur maculaire centrale est plus importante chez les patients injectés par brolucizumab-BEOVU©. La présence de liquide à la semaine 12 et 20 et le nombre d’IVT anti-VEGF entre la semaine 12 et 20 sont similaires entre les deux groupes. Cependant, à la semaine 16, l’absence de liquide est plus importante chez les patients injectés par brolucizumab-BEOVU©. Conclusion : La molécule anti-VEGF brolucizumab-BEOVU© a une efficacité similaire à celle de la molécule aflibercept-EYLEA© dans le traitement de la DMLA exsudative.
