How to build bridges? : The specialist nurses' experiences of teamwork in emergency care of patients in municipal health care

Abstract
Bakgrund: God vård kännetecknades av patienters delaktighet, integritet och självbestämmande. Sjuksköterskor inom kommunal primärvård och ambulanssjukvård möts i hemmen hos svårt sjuka patienter. Möten mellan sjuksköterskeprofessioner vid akut omhändertagande av patienter, ställer höga krav på tydlig kommunikation, samordnat teamarbete och ett etiskt förhållningssätt. Detta gäller i samarbete mellan olika professioner, samt i möten mellan sjuksköterskor och patienter, vilket i sin tur främjar och stärker personcentrerad vård. Syfte: Syftet med studien var att belysa specialistsjuksköterskors upplevelser av teamarbete vid akut omhändertagande av patienter inom kommunal primärvård. Metod: En kvalitativ intervjustudie med induktiv ansats användes. Tio individuella semistrukturerade intervjuer genomfördes med sjuksköterskor från kommunal primärvård och ambulanssjukvård. Datamaterialet analyserades med kvalitativ innehållsanalys enligt Graneheim och Lundman. Resultat: Ur analysen av datamaterialet framträdde ett tema; förutsättningar och teamarbete påverkar det akuta omhändertagandet, och tre kategorier; dokumentation och kommunikation säkerställer patientsäkerheten, samarbete påverkas av stress och etiskt förhållningssätt, samt organisatoriska faktorer och personliga egenskaper. Konklusion: Samarbete mellan sjuksköterskor präglades av strukturerad kommunikation, etiskt och respektfullt förhållningssätt samt en vilja att lita på varandra. När dessa förutsättningar fanns, banas väg för teamarbete samt samordnad och trygg vård med patienter i fokus
