L’impact neurologique de la sténose carotidienne chez les patients subissant une chirurgie cardiaque sous circulation extra-corporelle

Abstract

CONTEXTE : L’accident vasculaire cérébral (AVC) est une complication péri-opératoire redoutable des chirurgies cardiaques. En période post-opératoire, le risque neurologique attribué à la sténose carotidienne des patients devant subir une chirurgie cardiaque sous circulation extra-corporelle (CEC) demeure cependant méconnu. L’objectif de cette étude est donc d’évaluer l’association entre l’incidence d’AVC et d’ICT post-opératoires et la maladie carotidienne selon les diverses caractéristiques de la sténose (degré, latéralité et symptomatologie associée).
\nOBJECTIFS : Nous avons effectué une étude rétrospective de cohorte incluant les patients ayant subi une chirurgie cardiaque sous CEC à l’Institut Universitaire de Cardiologie et de Pneumologie de Québec entre janvier 2002 et mars 2018. Les variables pré-, per- et post-opératoires ont été recueillies à l’aide de la banque de données de l’établissement ainsi qu’à l’aide des dossiers. Une analyse univariée et une régression logistique multivariée par approche Stepwise furent effectuées.
\nRÉSULTATS : Au total, 26,845 patients respectant les critères d’inclusion ont subi une chirurgie cardiaque sous CEC à l’IUCPQ entre janvier 2002 et mars 2018. Parmi ces patients, 625 (2.38%) présentaient une sténose carotidienne (SC) de ≥50% unilatérale ou bilatérale. L’analyse multivariée démontre que chaque degré de sévérité (50-79%, 80-99%, 100%) de la sténose carotidienne représente un facteur de risque indépendant d’AVC et/ou d’ICT post-opératoires. On observe une association positive entre le degré de sévérité de la sténose carotidienne et l’incidence d’AVC et/ou d’ICT post-opératoire : SC 80-99% (10.2%) vs SC 50-79% (5.5%). Ni la latéralité (p=0.5488) de la sténose carotidienne ou la symptomatologie (p=0.1302) n’ont démontré d’association significative avec le risque neurologique post-opératoire.
\nCONCLUSION : Ces résultats démontrent que la sténose carotidienne est un facteur de risque indépendant d’AVC et/ou d’ICT post-opératoire. Il y a également une association positive entre le degré de sévérité de la sténose carotidienne et l’incidence des évènements neurologiques post-opératoire. Ce potentiel lien de cause à effet serait à explorer dans une étude prospective ultérieure.
