Le Modulen® dans la maladie de Crohn, étude de 60 patients au CHU de Rouen

La maladie de Crohn appartient au groupe des MICI Maladies Inflammatoires Chroniques Intestinales. Au cours des poussées de la maladie, il est possible d'avoir recours à un traitement nutritionnel. Le Modulen® est un de ces traitements, spécifiquement indiqué de part sa composition enrichie en cytokines anti-inflammatoires TGFb2 et son efficacité, dans la dénutrition associée à la maladie de Crohn. En pratique, il est utilise également dans d' autres indications, notamment dans les cas de corticorésistance ou dépendance, de projet chirurgical mais aussi lorsque les autres traitements sont contre-indiqués. Les objectifs de notre étude étaient d évaluer l efficacité et la tolérance du Modulen® dans ces indications, ainsi que de rechercher des facteurs associés au succès. Notre étude réalisée sur 60 patients porteurs d'une maladie de Crohn a montré une efficacité du Modulen ® dans 80% des cas, toute indication confondue. Il n'existait pas de différence de succès selon l'indication. Le taux de succès était statistiquement associé au phénotype de la maladie. Les formes sténosantes et pénétrantes (B2 et B3 selon la classification de Montréal) sont associés à de meilleurs résultats (90.2%) par comparaison aux formes B1 (66.7%) non pénétrantes et non sténosantes (p=0.04). L efficacité du Modulen® ne semblait pas être influencé par les autres caractéristiques des patients. II existait une tendance à des traitements plus prolongés en cas de succès 5.5 +-5 semaines versus 4 +-5.2 en cas d échecs (p=0.08). Le Modulen® administré par vole orale a été bien toléré pour 85% des patients. Cette étude conforte donc l'utilisation du Modulen® dans le traitement de la dénutrition au cours de la maladie de Crohn mais suggère également son intérêt dans d'autres indications telles que la corticorésistance ou dépendance, la préparation avant un projet chirurgical, ou l'existence d'une contre indication temporaire aux autres traitements de la maladie. Son efficacité parait plus importante dans le sous groupe des patients phénotype B2-B3.
