Periprothetische Femurfrakturen - der aktuelle Stand zu Literatur, Klassifikation und Therapie

Abstract
Durch die immer älter werdende Gesellschaft und der dadurch steigenden Prävalenz von Hüftgelenkstotalendoprothesen steigt auch die Inzidenz an periprothetischen Frakturen des Femurs und die Notwendigkeit der Versorgung ebendieser. \nIn der folgenden Arbeit wurde ein Überblick über die Einteilung und Versorgungsmöglichkeiten periprothetischer Femurfrakturen erstellt. Dabei wurden mithilfe von Literatur aus den letzten 15 Jahren die gängigsten Formen der Frakturversorgung, verschiedene Osteosyntheseverfahren sowie Schaftwechsel, verglichen und versucht eine optimale Behandlungsmethode, bezogen auf die Vancouver Klassifikation, zu finden. \nFrakturen vom Typ A sollen demnach, außer bei starker Dislokation der Frakturfragmente bei welcher eine Plattenosteosynthese erfolgen sollte, konservativ mit Teilbelastung versorgt werden. Typ B1 sowie C sollte mittels Osteosynthese, vorzugsweise mit winkelstabilen Platten, und Typ B3 mittels Schaftwechsel versorgt werden. Bei Frakturen vom Typ B2 kann eine Versorgung mittels Schaftwechsel oder Osteosynthese erfolgen, wobei die Ergebnisse bei Versorgung mittels Osteosynthese, bei durchschnittlich kürzerer Operationszeit und geringerem Blutverlust, teilweise ein schlechteres Outcome in Bezug auf die Mobilität der Patientinnen und Patienten zeigt. \nInsgesamt wurde über alle Frakturtypen hinweg eine hohe Rate an Komplikationen und Mortalität verzeichnet, was auf das hohe Alter der Patientinnen und Patienten bei Frakturgeschehen zurückzuführen ist.
