آيا تابش پروفيلاکتيک ليزر PDL 585 nm در بهبود پيش اسکارهای (prescars ) ماموپلاستی موثر است؟

Abstract
زمينه: در طی دو دهه اخير از تابش نور ليزر Pulsed – dye laser (PDL) در درمان اسکارهای هيپرتروفيک و کلوئيد استفاده شده که با درجات مختلفی از موفقيت همراه بوده است. بعضی از مطالعات، بر درمان زودرس اسکار به وسيله PDL جهت دستيابی به نتيجه بهتر تأکيد دارند، لذا محققين از PDL جهت پيشگيری از ايجاد اسکار هيپرتروفيک و کلوئيد در محل انسيزيونهای جراحی در مراحل اوليه ترميم زخم استفاده کردهاند. اين کارآزمايی بالينی تصادفی شده با گروه کنترل، به منظور بررسی اثر استفاده پروفيلاکتيک (سه هفته بعد از عمل جراحی) از PDL 585 nm در کاهش شدت اسکارهای جراحی در انسيزيونهای reduction ماموپلاستی دو طرفه در مراجعهکنندگان به مرکز تحقيقات ليزر پزشکی جهاد دانشگاهی علوم پزشکی تهران طرح ريزی و اجرا شد. \n \n روش بررسی: در اين طرح 6 بيمار که تحت reduction ماموپلاستی دو طرفه قرار گرفته بودند، مطالعه را به پايان رساندند. در هر بيمار 3 سانتی متر از انتهای مديال و 3 سانتی متر از انتهای لترال خط انسيزيون به صورت تصادفی به عنوان کنترل انتخاب و قرينه آن در طرف مقابل، تحت تابش با PDL 585nm قرار ميگرفت ( در مجموع 24 ناحيه اسکار، شامل 12 منطقه کنترل و 12 منطقه ليزر). تابش ليزر به فواصل 1 ماه، در 2 نوبت ديگر با همين پارامترها تکرار شد. ارزيابی اسکارهای بيماران قبل از انجام پروسه و نيز طی زمانهای پيگيری يعنی انتهای ماه اول و دوم (قبل از انجام ليزر) و 2 ماه بعد از آخرين جلسه تابش ليزر، توسط پزشک مستقل با استفاده از VSS (Vancouver Scar Scale) صورت گرفت. \n \n يافتهها: دو ماه بعد از سومين جلسه تابش، ضايعات گروه ليزر، بهبودی قابل توجهی را از نظر واسکولاريتی، انعطاف پذيری و ارتفاع نشان ميدادند (p value< 0.001) . در حالی که در گروه کنترل، ميزان بهبود واسکولاريتی (p value=0.14) ،انعطاف پذيری (p value=0.13) معنی دار نبود و ارتفاع اسکارها تغييری را نشان نمی داد. \n \n نتيجهگيری: نتايج اين کارآزمايی بالينی تصادفی شده نشان ميدهد که استفاده پروفيلاکتيک از سه جلسه تابش ليزر PDL 585 nm با دوز پايين، ميتواند به عنوان روشی مناسب و در عين حال ايمن در بهبود واسکولاريتی، انعطاف پذيری و کاهش ارتفاع اسکار ناشی از خط برش reduction ماموپلاستی ) با انسيزيون inverted-T ( محسوب گردد.
