[Résumé français]L'entorse de cheville constitue un traumatisme très important, en France, de par sa fréquence, ses risques de séquelles et son coût. Les protocoles d'Ottawa, établis par le Docteur Stiell, visent à améliorer la prise en charge de ces traumatismes et à réaliser des économies de santé et de temps. Notre étude a consisté à analyser les modalités de prise en charge des entorses de cheville dans deux SAU parisiens, Bichat et Kremlin Bicêtre. Nous avons réuni une population de 407 personnes sur une période de 29 jours. Il s'est dégagé de cette étude que la prise en charge était globalement bonne et pratiquement identique dans les deux hôpitaux. Cependant, si les règles d'Ottawa avaient été appliquées, ce qui n'est pratiquement jamais le cas, un tiers des clichés radio aurait pu être évité, constituant une économie substantielle, et un gain de temps non négligeable dans la prise en charge des patients. Nous avons également entrevu, au cours de cette étude, que d'autres économies de santé publique, auraient pu être réalisées, notamment, en favorisant le remboursement de la location de cannes anglaises plutôt que leur achat. Ces constats ouvrent la voie à la réalisation d'économies publiques très substantielles, aisées, sans diminution de la qualité des soins, ce qui est toujours d'actualité en matière de santé publique.
