Asociación entre la alexitimia y la adherencia al tratamiento antirretroviral de gran actividad (targa) en personas adultas con infección por VIH/SIDA

Abstract
Introducción: Se define como alexitimia a la incapacidad, déficit o deficiencia para identificar y expresar sentimientos. El objetivo fue evaluar si existe asociación entre la Alexitimia y la adherencia al TARGA en personas con VIH/SIDA. Métodos: Llevamos a cabo un estudio transversal analítico en 561 pacientes en Lima, Perú. El riesgo de Alexitimia(RA) fue medido con la escala de Alexitimia de Toronto(TAS-20) y la adherencia al TARGA con el CEAT-VIH. Medimos edad, sexo, estado civil, orientación sexual, esquema de TARGA, tiempo de enfermedad, y riesgo de depresión. Desarrollamos modelos de regresión logística multinomial con errores estándar robustos para estimar Razones de Prevalencia (RP) e IC95%. Resultados: El 27,4% poseían RA y un 24% adherencia baja. La media de tiempo de enfermedad fue 87,3 meses. En el análisis bivariado hallamos asociación entre la mala adherencia y RA (p< 0,002), riesgo de depresión (p<0,001), esquema TARGA (p=0,04), edad (p=0,02), tiempo de enfermedad (p=0,01) y riesgo de tabaco (p=0,02). Encontramos asociación entre el RA y la baja adherencia tanto a nivel crudo (RP: 1,96 IC95% 1,30 a 2,95) como ajustado, por esquema TARGA, tiempo de enfermedad y edad (RP:1,84 IC95% 1,21 a 2,82). Conclusiones: El RA se asocia a baja adherencia al TARGA en la población estudiada independientemente de otras variables clínicas y demográficas. La exploración y manejo de esta condición resulta relevante en la práctica clínica.
