OBJECTIFS : évaluer les compétences des médecins généralistes en communication dans une situation de handicap partagée. MÉTHODE : enquête par questionnaire auprès d un panel de 47 sourds signants en Île-de-France de mars 2009 à décembre 2009, explorant la compétence en communication du médecin traitant lors de la dernière consultation au travers des tâches telles qu elles sont décrites dans la grille de Calgary Cambridge. RESULTATS : Les pratiques communicationnelles étaient majoritairement verbales pour les sourds et les médecins, par contre les sourds mobilisaient des stratégies non verbales 3 fois plus que les médecins. 82,3% des personnes interrogées estimaient que le médecin adaptait sa communication en fonction de leur surdité : maintien du contact visuel (64%), recours à la patience (48,7%), vérification de la compréhension (41%), bonne articulation labiale (43,6%), simplification des écrits (35,9%). Les stratégies de reformulation étaient très peu utilisées (12,8%), de même que les supports visuels (5%). Globalement, les personnes avaient estimé comprendre les explications données sur le diagnostic (90%), le traitement (89,7%) et les examens complémentaires (85,7), mais peu de résumés étaient mentionnés en fin de consultation (30,9%). Les critiques portaient principalement sur le manque d'intégration dans la prise de décision les concernant (61,6%), le manque de respect du droit à l'information (54,4%) et l'utilisation importante du jargon médical (44,4%). CONCLUSION : Les personnes sourdes estimaient être satisfaites de la relation et de la consultation. Mais 30 % d'entre eux étaient insatisfaits de la communication établie avec leur médecin.
