Efficacy of SSRI antidepressants according to the type of interaction with tamoxifen in women with ER+ breast cancer diagnosed with depression, a systematic review

Abstract
Objetivos: evaluar la eficacia de los antidepresivos ISRS de acuerdo al tipo de interacción con tamoxifeno en mujeres con cáncer de mama RE+ y diagnóstico de depresión. Comparar la concentración sérica del metabolito activo del tamoxifeno, endoxifeno. Materiales y métodos: se realizó una revisión sistemática en las bases PubMed, Cochrane, SCOPUS, EBM (Ovid), OVID MEDLINE, Google Scholar, Lilacs, Web of Science, Clinical Trials, EBSCO y EMBASE hasta enero del 2023. Se incluyeron estudios de tipo cohorte que evalúen a mujeres con cáncer de mama RE+ en estadio temprano (I y II) en tratamiento con tamoxifeno y con diagnóstico de depresión. El proceso de revisión de datos se realizó en dos fases. Se evaluó el riesgo de sesgo mediante la herramienta New-Castle Ottawa. Se combinaron los resultados cuantitativos de los estudios cohorte mediante un forest plot siguiendo modelos de efectos aleatorios para abordar mejor la heterogeneidad de las características de la población entre los estudios. Resultados: se incluyeron tres estudios cohorte. Se encontró que las mujeres con cáncer de mama que consumen antidepresivos con fuerte interacción tienen menor concentración de endoxifeno en 7.64 ng/ml, p=0.001 IC 95% (-12.23, -3.05). En uno de los estudios la concentración final de endoxifeno no superó el umbral de respuesta terapéutica. Conclusiones: no se puede afirmar que los antidepresivos de leve interacción ocasionan mejoría de síntomas depresivos y mayor concentración de endoxifeno con la evidencia disponible.
