Contexte : Les nourrissons avec bronchiolite virale aiguë sévère requièrent fréquemment un support respiratoire nasal (SRN) tel une pression positive continue nasale (PPCn) ou des lunettes nasales à haut débit (LNHD). L'alimentation orale de ces nourrissons sous SRN demeure controversée par crainte d'aspirations menant à des événements cardiorespiratoires, tels que des apnées et/ou bradycardies et/ou désaturations. L’objectif principal de cette étude était d’évaluer la sécurité de l’alimentation orale sous différents SRN chez un modèle ovin de nouveau-né à terme. Méthodes : huit agneaux âgés de 4 à 5 jours ont été étudiés. Une instrumentation chirurgicale était réalisée sous sédation consciente et anesthésie locale afin d’enregistrer la succion, la déglutition, la respiration, l'ECG, la saturation et les gaz artériels (cathéter carotidien). Chaque agneau prenait sous vidéofluoroscopie 60 mL de lait, soit 2 biberons de 30 mL séparés par une pause de 30 secondes, dans quatre conditions randomisées [PPCn 6 cmH 2 0, LNHD 7 L/min, LNHD PPCn et contrôle (sans SRN)]. L’étude se déroulait sur deux journées randomisées, avec et sans tachypnée standardisée induite par compression thoracique (brassard à tension). Des modèles linéaires généralisés multivariés ont permis de comparer les 4 SRN sur le plan de la sécurité (événements cardiorespiratoires et aspirations), la coordination succion-déglutition-respiration et l’efficacité de l’alimentation orale. Résultats : Des pénétrations laryngées et/ou aspirations trachéales sont survenus au cours de 6 séances d’alimentations au biberon parmi les 101 enregistrées sous vidéofluoroscopie. Bien qu’aucune différence statistiquement significative n’ait été retrouvée entre les SRN, 97% (104/107) des pénétrations laryngées ont été observées en condition LNHD ou LNHD PPCn . La condition LNHD a significativement altéré la coordination succion-déglutition-respiration sans toutefois compromettre la sécurité de l’alimentation orale. Conclusion : Nos résultats suggèrent que l'alimentation orale sous SRN est sécuritaire chez un nouveau-né à terme, même avec tachypnée. Ces résultats justifient de procéder à une étude similaire chez des agneaux prématurés, pour tester la sécurité de l’alimentation orale sous SRN en présence d’une immaturité du système nerveux.
