Der Ausbruch der Coronavirus-Pandemie stellte weltweit eine bedeutende Herausforderung dar. Krankenhäuser waren mit der Ausbreitung und Übertragungsgefahr von COVID-19 (coronavirus disease 2019) konfrontiert. Dies betraf auch das Verfahren der Transkatheter-Aortenklappenimplantation (TAVI) für Patientinnen und Patienten mit hochgradiger symptomatischer Aortenklappenstenose. Zur angepassten Auswahl und Priorisierung von TAVI-Patientinnen und TAVI-Patienten während der Pandemie etablierten wir einen sogenannten Allokationsalgorithmus, der auf den Empfehlungen der American College of Cardiology / Society for Cardiovascular Angiography and Interventions (ACC / SCAI) und denen der European Society of Cardiology (ESC) beruhte. Um zu prüfen, ob durch eine erfolgreiche Implementierung dieses Allokationsalgorithmus eine pandemieadaptierte TAVI-Patientinnen- und TAVI-Patienten-Priorisierung gelang, und zu analysieren, welche Auswirkungen der Einsatz dieses Allokationsalgorithmus auf prä-, intra- und postprozedurale Parameter hatte, führten wir folgenden Vergleich durch: 456 Patientinnen und Patienten, welche vor der Pandemie und somit vor der Implementierung dieses Allokationsalgorithmus (prä-COVID-19-Gruppe) eine TAVI erhielten, wurden 456 Patientinnen und Patienten gegenübergestellt, bei denen eine TAVI zu Pandemiebedingungen und dadurch mit Nutzung unseres Allokationsalgorithmus durchgeführt wurde (COVID-19-Gruppe). Es konnten mehrere signifikante Unterschiede festgestellt werden: Die COVID-19-Gruppe zeigte in Bezug auf Patientencharakteristika eine höhere Rate an kardialen Dekompensationen/ kardiogenen Schocks (10,5% vs. 1,3%; p< 0,001), Blockbildern (Linksschenkelblock: 10,9% vs. 4,8%; p= 0,001), schwerer symptomatischer Angina pectoris (Canadian Cardiovascular Society (CCS) II, III, IV: 18,7% vs. 11,8%; p= 0,004), Troponinerhöhung (>14 ng/L: 84,9% vs. 77%; p= 0,003) und reduzierter linksventrikulärer Ejektionsfraktion (LVEF) (<45%: 18,9% vs. 12%; p= 0,006). In der Pandemie waren mehr Prädilatationen (46,3% vs. 35,1%; p= 0,001) und eine längere Prozedurdauer (80,2 min (+/-29,4) vs. 66,9 min (+/-17,5); p< 0,001) erforderlich, wobei sich der Erfolg der Klappenimplantation nicht signifikant von der prä-COVID-19-Gruppe unterschied (0% vs. 0,4%; p= 0,17). Während COVID-19 war der Krankenhausaufenthalt kürzer (8,4 Tage (+/-5,9) vs. 9,5 Tage (+/-9,33); p= 0,041) und eine geringere Anzahl an Patientinnen und Patienten pro Monat wurde einer TAVI-Prozedur unterzogen (39 (+/-4,55) vs. 46,11 (+/-7,57); p= 0,03). Zusammenfassend hat die Implementierung unseres Allokationsalgorithmus geholfen, in der COVID-19-Pandemie Patientinnen und Patienten in einem kardial kritisch kranken Zustand für die TAVI-Prozedur zu priorisieren. Die Prozedur während COVID-19 konnte, trotz höherer Komplexität, mit einem vergleichbaren Erfolg durchgeführt werden. In der Pandemie wurden weniger TAVI-Prozeduren pro Monat mit kürzerer Krankenhausaufenthaltsdauer absolviert.
