Le système de santé et des services sociaux québécois en est à sa troisième grande réforme administrative depuis sa conception. Ces changements majeurs ont des impacts sur les droits des usagers, principalement sur l’accessibilité et sur la qualité des soins et services. Les réformes antérieures et celles faites ailleurs dans le monde nous enseignent que l’accessibilité aux soins et services est diminuée à la suite des ces restructuration. En effet, les points d’accès sont rationalisés et les parcours de soins des usagers sont complexifiés parce que les mécanismes de référence prennent du temps à se mettre en place et les endroits où sont dispensés les soins changent également. D’autre part, la qualité des soins est souvent reléguée en second plan lors de l’implantation des réformes administrative puisque les gestionnaires et les employés doivent mettre beaucoup de temps et d’énergie pour mettre les changements en place, ce qui diminue beaucoup le temps qu’ils peuvent consacrer directement aux usagers ou le temps utilisé pour la formation et le développement des pratiques professionnelles. L’informatisation du réseau de la santé et des services sociaux et le travail interdisciplinaire sont des bons exemples qui peuvent améliorer l’accès et la qualité des soins et services.
