La prise en charge pré-hospitalière des arrêts cardio-respiratoires sur le secteur du SMUR de Rouen est évaluée par une étude prospective menée du lei janvier au 31 décembre 1996. Elle porte sur 405 dossiers dont 398 arrêts cardio-respiratoires confirmés par le SMUR. Parmi ces patients, 12 sortiront vivants de l'hôpital dont 8 seront encore en vie 1 an après leur arrêt cardiaque. Pour les patients trouvés en fibrillation ventriculaire à la suite d'un arrêt cardiorespiratoire en présence de témoins (population de référence proposée par le style d'UTSTEIN pour les comparaisons inter-structures), l'efficacité de la chaîne de survie est faible sur notre secteur. Les raisons en sont essentiellement la lenteur du déroulement de la chaîne de survie, le manque de formation du grand public responsable du faible taux de réanimations cardiopulmonaires de base effectuées par les spectateurs, l'absence de défibrillateur semiautomatique au premier niveau de réponse. L'amélioration du pronostic suppose la correction de ces points faibles et implique un raccourcissement des temps de réponse du système. L'analyse en continue de la chaîne de survie est indispensable pour évaluer sur de grandes séries l'efficacité des améliorations apportées.
