RATIONELLE: Dans les essais cliniques contrôlés randomisés, le choix de la pratique 
\ncourante comme comparateur peut être associé à une meilleure acceptabilité du protocole de 
\nl’étude par les cliniciens et mener à des résultats plus généralisables. Par contre, si la pratique 
\ncourante change en cours d’étude, l’écart entre les groupes peut changer. Nous avons mesuré 
\nla pratique courante avant et pendant un essai contrôlé randomisé, l’étude OVATION-65,
\névaluant de nouvelles cibles de pression artérielle moyenne guidant l'administration de
\nvasopresseurs. MÉTHODOLOGIE: L'essai randomisé contrôlé OVATION-65 a inclus des 
\npatients âgés de ³ 65 ans, admis aux unités de soins intensifs pour recevoir la pratique 
\ncourante ou un traitement restrictif de vasopresseurs, visant une pression artérielle moyenne 
\nde 60-65 mmHg, pour déterminer l’effet sur l’atteinte des organes cibles. Afin de déterminer 
\nsi l’initiation de l’étude a engendré des changements de la pratique courante, nous avons 
\nmené une étude rétrospective observationnelle dans laquelle nous avons évalué la pression 
\nartérielle moyenne atteinte de patients avant l’initiation de l’étude et pendant son 
\ndéroulement en utilisant un test-t de Student non apparié. RÉSULTATS: Nous avons inclus 
\n200 patients avant et 229 patients après l’initiation d’OVATION-65. Les caractéristiques de 
\nbase des deux groupes étaient comparables (score Acute Physiology and Chronic Health 
\nEvaluation II [APACHE II], médiane 26 [intervalle interquartile (IIQ) 20-31] vs 26 [IIQ 21-
\n31], p=0,53 et âge moyen, 74,5 [écart-type (ET) 7,2] vs 75,2 [ET 6,9] ans, p=0,28). Nous 
\nn’avons pas observé de différences significatives entre les pressions artérielles moyennes
\natteintes moyennes avant et après l’initiation d’OVATION-65 (72,5 [ET 5,1] vs 72,4 [ET
\n5,0] mmHg, p=0,76). CONCLUSION: L’initiation de l’essai contrôlé randomisé
\nOVATION-65 n'a pas été associée à un changement des valeurs de pression artérielle 
\nmoyenne atteinte et n’a donc pas entraîné la modification de la pratique des cliniciens en 
\nlien avec l'administration des vasopresseurs. Pour les essais contrôlés randomisés comparant 
\nun traitement actif à la pratique courante, l’observation délibérée des pratiques courantes 
\navant et pendant l’essai clinique est importante pour détecter des changements aux pratiques 
\ncourantes pouvant être liés à la l’initiation de l'étude et pourrait atténuer les inquiétudes 
\nconcernant les biais potentiels ou permettre de quantifier les biais.
