Bakgrund: Intensivvårdsavdelningen är en påfrestande miljö vilket kan försvåra kommunikationen i vårdteamet. Hierarki är en av faktorerna som kan påverka kommunikationen, genom en jämlik kommunikation, där alla i teamet får ta plats och bli hörda kan detta i sin tur bidra till en mer patientsäker vård. Syfte: Syftet med studien var att beskriva kommunikationen i vårdteamet kring en kritiskt sjuk patient på en intensivvårdsavdelning. Metod: En kvalitativ, induktiv ansats har använts till studien. På en intensivvårdsavdelning i Sverige gjordes 19 stycken icke deltagande, ostrukturerade observationer av kommunikationen i vårdteamet. Den insamlade datan analyserades sedan med en fenomenologisk hermeneutisk metod enligt Lindseth och Norberg (2004). Resultat: Två teman och fyra subteman framkom ur analysen. “Kommunikationen påverkas av relationer” visar hur samspelet i vårdteamet formas genom yrkesroller och interaktionen mellan personalen. Samspelet påverkade hur teammedlemmarna kommunicerar och förstår varandra. I temat “Kommunikation kan både hjälpa och stjälpa” framkom hur kommunikationen fungerade i det vardagliga arbetet. Det beskrev olika kommunikationsmönster och vad som kan störa kommunikationen inom vårdteamet. Konklusion: Studien visade att kommunikationen i vårdteamet påverkades av relationer, hierarki och uttryckssätt. Strukturer kunde både underlätta och hindra öppenhet samt förståelsen.
