Introducción: La enfermedad de Alzheimer (EA), trastorno neurodegenerativo, y por tanto, progresivo e irreversible, es la forma más frecuente de demencia; síndrome altamente incapacitante. No obstante, esta es la última etapa de un Continuum neurodegenerativo. En este marco biológico, se emplean biomarcadores para establecer la definición de EA, ofreciendo la posibilidad de un diagnóstico precoz. Objetivo: Evaluar la eficacia de los marcadores biológicos sanguíneos en el diagnóstico de la EA esporádica durante la fase de predemencia. Metodología: Se realizó una búsqueda bibliográfica sistemática en PubMed y Scopus que posteriormente fue cribada al aplicar los criterios de inclusión y exclusión. Los datos más relevantes fueron agrupados en una tabla resumen. Para evaluar la calidad de la evidencia se emplearon los niveles de evidencia del Scottish Intercollegiate Guidelines Network y para valorar el riesgo de sesgos la Newcastle-Ottawa Scale y el QUADAS-2. Resultados: Se evaluaron 11 estudios publicados en los últimos 10 años, reuniendo un total de 6.555 participantes. Se evidenció la asociación de los biomarcadores plasmáticos afamina, IGHM, BACE1, cociente Aβ1-42/Aβ1-40, ptau181, GFAP, NFL, sAXL, IL-6 e YKL-40 con diferentes características de la EA. Asimismo, la mayoría mostró potencial tanto en el diagnóstico precoz de la EA como en la predicción de progresión a demencia por EA. Conclusión: El cociente Aβ1-42/Aβ1-40, p-tau181 y NFL son los biomarcadores plasmáticos que han exhibido mayor potencial para ser aplicados en la práctica clínica y detectar a pacientes con EA esporádica en la etapa prodrómica.
