Birinci basamakta orta-ağır seviyeli osteoartritli hastalarda oral kolşisintedavisi: randomize, çift-kör, faz III klinik çalışma

Abstract
Giriş Birinci basamak hasta popülasyonunun önemli bir kısmını osteoartritli hastalar oluşturmaktadır. Türkiye’de yapılan bir çalışmada diz osteoartrit (OA) prevalansı %5.351, el OA prevalansı %1.11, toplam OA prevalansı %8.42 olarak rapor edilmiştir. Tedavi seçenekleri arasında ise semptom gidericiler olarak kullanılan ağrı kesiciler bulunmaktadır. Patogenezinde kalsiyum pirofosfat kristallerinin rol oynadığının gösterilmesiyle kalsiyum içeren kristaller ile oluşan inflamasyonda (psödogut) yararlı olduğu gösterilen kolşisinin tedavide kullanılması gündeme gelmiştir. Kolşisin gibi maliyeti düşük ve çok eski bir molekülün osteoartrit tedavisinde hem semptom giderici hem de hastalık modifiye edici etkisinin olabileceği düşünülmektedir. Biz de kendi hasta grubumuzda kolşisinin hastaların ağrı, tutukluk ve fonksiyonları üzerindeki etkilerini WOMAC (Western Ontario and McMaster Universities Osteoarthritis Index) skalasını kullanarak araştırmayı amaçladık. Yöntem Çalışma evrenini, İstanbul sınırlarında belirlenecek bir Aile Sağlığı Merkezi’ne başvuran 40-65 yaş arasındaki ACR (American College of Rheumatology) OA kriterlerine uyan, Oxford Diz Skoru 20- 29 olan orta-ağır derecede osteoartritli gönüllü hastalar oluşturmaktadır. Gönüllü hasta sayısı-50 hasta kolşisin, 50 hasta plasebo grubunda olmak üzere-100 olarak belirlenmiştir. Tüm hastalara 4 hafta boyunca 3x1000 mg parasetamol tablet peroral verilecektir. 4 hafta sonra kolşisin grubuna 2x0.5mg kolşisin film tablet, plasebo grubuna 2x0.5mg nişastayla doldurulmuş film tablet 3 ay tedavi süresi boyunca peroral verilecektir ve her iki grup parasetamol tedavisine devam edecektir. Vizitler 1., 2., 3., 4. ayın sonunda yapılacaktır ve her vizitte hastalar muayene edilecek, şikayetleri sorgulanacak, WOMAC ve VAS (visual analogue scale) uygulanacaktır. Bulgular Çalışmanın primer sonlanım noktası WOMAC’taki, sekonder sonlanım noktası VAS’taki gelişmedir. Tanımlayıcı istatistiklerin yanı sıra karşılaştırmalı analizlerde ki-kare testi, Student’s t test; parametrik varsayımlar karşılanmadığı durumda Mann-Whitney-U test, her bir grubun öncesi ve sonrası karşılaştırmalarında Paired Sample t-test veya non-parametrik eşdeğeri Wilcoxon testi kullanılacaktır.
