Challenges of handover in a postoperative care unit : An observation study with a qualitative approach

Abstract
Bakgrund: Bristande kommunikation är en vanlig orsak till vårdskador, vilket gör tydlig överrapportering mellan vårdprofessioner avgörande för patientsäkerheten. Effektiv kommunikation minskar risken för missförstånd och bidrar till en säkrare vårdmiljö. Syfte: Syftet med studien var att undersöka överrapportering av patienter från operation till postoperativ avdelning mellan anestesi- och intensivvårdssjuksköterskor. Metod: En kvalitativ ansats ligger till grund för studien och en ostrukturerad icke-deltagande observationsstudie har genomförts. Totalt observerades 31 överrapporteringar. Materialet från observationerna har analyserats utifrån Elo och Kyngäs induktiva kvalitativa innehållsanalys. Resultat: Fem subkategorier och två kategorier framkom efter analysen. I kategorin samverkan i team belyses styrkor och utmaningar för överrapporteringens struktur, genomförande samt hur de påverkar arbetsflödet. I kategorin utmaningar vid överrapportering uppdagades att buller, pipande ljud, kollegor som samtalande högt, patienters fysiska och emotionella tillstånd kunde skapa hinder för en effektiv överrapportering. Konklusion: Studien betonar vikten av strukturerad kommunikation och samarbete mellan anestesi- och intensivvårdssjuksköterskor för att säkerställa effektiv och säker överrapportering. Användningen av kommunikationsverktyg minskar risken för missförstånd, men kräver en störningsfri arbetsmiljö för att vara effektiv. För att möta utmaningarna behövs organisatoriska förbättringar, såsom anpassad arbetsmiljö och åtgärder för att minska störningar.
