Contributo à compreensão de uma patologia incompreendida : a fibromialgia na perspectiva psicanalítica da psicossomática

Abstract
A fibromialgia (FM) é uma síndrome de dor crónica que afecta uma percentagem considerável da população e em relação à qual não existe qualquer consenso. Através da junção de dados provenientes de três fontes distintas (observação clínica, teoria psicossomática psicanalítica, e investigação científica), foi desenhada um estudo exploratório cujo objectivo geral era analisar a possibilidade de conceptualizar a FM enquanto perturbação psicossomática, definida operacionalmente no quadro da psicossomática psicanalítica. A amostra foi composta por 42 sujeitos do sexo feminino, que tinham recebido um diagnóstico de FM sem outra perturbação somática associada, com idades compreendidas entre os 33 e os 71 anos. Todos os sujeitos preencheram um protocolo composto por: um questionário para caracterização da amostra, o Inventário de Convicções e Percepções Relacionadas com a Dor (ICPRD), o Desenho de Círculos Representativos da Família (DCRF), a Escala de Alexitimia de Toronto de 20 Itens (TAS-20), a Positive and Negative Affect Schedule (PANAS), e um novo instrumento, o Inventário do Funcionamento Onírico. Estabeleceu-se que seriam necessários três indicadores distintos para se considerar a presença de perturbação psicossomática em cada sujeito: (1) indícios de perturbação ou trauma da relação precoce; (2) um resultado superior a 51 na TAS-20; e (3) um resultado superior a 2 no IFO. 83.3% (n = 35) dos sujeitos que compunham a amostra apresentaram resultados que permitiram considerar confirmada a hipótese geral: os pacientes com diagnóstico de FM apresentam um conjunto de características no seu funcionamento mental compatíveis com a existência de perturbação psicossomática, tal como descrita na teoria psicossomática psicanalítica. As implicações destes resultados, bem como de outros obtidos nesta investigação, são discutidas.
