Introducción: La infección por coronavirus puede ocasionar SARS y MERS, con graves trastornos de oxigenación y requerimiento de manejo en UCI y terapias de rescate como ECMO. Objetivo: Establecer la mortalidad de pacientes con SARS o MERS llevados a ECMO en la unidad de cuidados intensivos. Métodos: Revisión sistemática mediante búsqueda en 4 bases de datos (Medline, Embase, Lilacs y Cochrane) y búsqueda manual. ECA, ECC y estudios analíticos fueron elegibles. Dos revisores tamizaron, extrajeron información de la evidencia y evaluaron la calidad metodológica independientemente usando la escala de Newcastle-Ottawa. El cuerpo de la evidencia se evaluó con la metodología GRADE. Resultados: Se identificaron 458 referencias; una cumplió criterios de elegibilidad, correspondiente a una cohorte retrospectiva, control histórico, con riesgo de sesgo y calidad de la evidencia bajos, que incluyó 35 pacientes (17 ECMO/18 control), con edades medianas de 45,5 y 50 años respectivamente, mayor porcentaje de hombres en ambos grupos y una mediana en la escala APACHE II de 27,8 y 31. ECMO se asoció significativamente con menor mortalidad a los 90 días (p=0,02) y mayor estancia mediana en UCI (22,5 Vs 7 días; p=0,001). En el grupo ECMO se reportaron dos sangrados mayores y un neumotórax. Conclusión: La evidencia que soporta el impacto de ECMO en la mortalidad de pacientes con MERS-CoV se limita a un estudio de bajo nivel de evidencia; el uso de ECMO se asoció significativamente con menor mortalidad a 90 días en pacientes con infección por MERS-CoV e hipoxia refractaria.
