La enfermedad pulmonar obstructiva crónica (EPOC) se caracteriza por inflamación de las vías respiratorias, obstrucción del flujo aéreo, hipoxia tisular y producción de sustancias oxidativas, que conducen a pérdida de masa muscular, desnutrición y perdida de la capacidad de realizar actividad física, desencadenando sarcopenia y/o fragilidad, como desenlaces negativos en sujetos con EPOC. Objetivo: Evaluar la interacción entre la EPOC, la fragilidad y la sarcopenia, y explorar los determinantes clínicos importantes en la EPOC. Materiales y métodos: Se realizó una revisión sistemática de la evidencia, se seleccionaron estudios transversales, de cohorte, casos y controles y estudios clínicos controlados. La calidad de los estudios se evaluó con la escala Newcastle Ottawa. Resultados: 520 artículos relacionaban la fragilidad y el EPOC y 466 artículos que relacionaron la sarcopenia con EPOC. Fueron seleccionados 12 y 16 artículos respectivamente. La prevalencia de fragilidad en EPOC es de 6.6% a 76%, siendo la EPOC un factor determinante para el desarrollo de fragilidad [OR 4,023 IC95% (2,53-6,36) P= 0,001]. La sarcopenia se presenta entre el 5,3% y 55% en los sujetos con EPOC. Clínicamente aquellos con EPOC, fragilidad y sarcopenia tenían mayor severidad de la disnea, peor capacidad de ejercicio y peor calidad de vida. Además, presentaban mayor número de exacerbaciones y mortalidad. Conclusiones: La sarcopenia y fragilidad son prevalentes en los sujetos con EPOC. Aquellos con EPOC tienen 4 veces más riesgo de desarrollar fragilidad, y aquellos que tienen fragilidad y sarcopenia presenta mayor disnea, disminución del desempeño físico y peor calidad de vida.
